* **Preoperative Sedation:** 2-4 mg IM given 2 hours before surgery.
* **Alcohol Withdrawal:** 2-4 mg orally or IM every 4-6 hours as needed. Dosing should be titrated based on symptom severity.
## Pediatric Dosing
Dosing in pediatric patients is highly variable and requires careful consideration of age, weight, and clinical condition. Specific dosing is often determined by institutional protocols or specialist guidance.
* **Status Epilepticus:** 0.1 mg/kg IV, not to exceed 4 mg per dose. May be repeated once after 5-10 minutes.
* **Sedation/Anxiolysis:** Doses range from 0.02-0.05 mg/kg orally or IV, not to exceed adult maximums.
## Dose Adjustments
* **Hepatic Impairment:** Use with caution; dosage reduction may be necessary.

* Signs of sedation and cognitive impairment.

## Clinical Pearls
* Lorazepam has a relatively long half-life and intermediate duration of action among benzodiazepines.

* Abrupt discontinuation can lead to withdrawal symptoms, including rebound anxiety, insomnia, irritability, tremors, and, in severe cases, seizures. Tapering is recommended for patients on prolonged therapy.
* Paradoxical reactions (e.g., agitation, aggression) can occur, particularly in children and the elderly.
