# Liquid Paraffin
## Overview
Liquid paraffin is a mineral oil used as an emollient and laxative. It works by lubricating the intestinal tract, softening the stool and making it easier to pass.
## Primary Indications
* Constipation
## Adult Dosing
* **Laxative:** 10-30 mL once daily, typically at bedtime.
* **Emollient:** Applied topically as needed.
## Pediatric Dosing
* **Constipation (Age 1-12 years):** 5-10 mL once daily, typically at bedtime. Dosing should be guided by clinical response and local protocol. Maximum daily dose generally not to exceed 20 mL.
* **Infants (under 1 year):** Generally not recommended due to increased risk of aspiration. Consult specialist if considered.
## Dose Adjustments
No specific dose adjustments are required for renal or hepatic impairment.
## Contraindications
* Appendicitis or symptoms of acute abdominal conditions.
* Intestinal obstruction.
* Hypersensitivity to liquid paraffin.
* Children under 1 year old (oral use due to risk of aspiration).
## Adverse Effects
* Anal leakage, abdominal cramping, diarrhea, nausea, vomiting, and perianal irritation.
* Long-term use may lead to vitamin malabsorption (fat-soluble vitamins A, D, E, K).
* Risk of lipoid pneumonia if aspirated.
## Key Drug Interactions
* Concurrent use with other mineral oil products can increase laxative effect and risk of adverse events.
* May reduce absorption of fat-soluble vitamins and other orally administered medications. Separate administration by at least 2 hours.
## Monitoring
* Monitor for bowel movements and stool consistency.
* Assess for signs of abdominal discomfort or leakage.
* Consider monitoring for fat-soluble vitamin deficiencies with long-term use.
## Clinical Pearls
* Administer at bedtime to minimize daytime discomfort.
* Avoid use in patients with dysphagia or an impaired gag reflex due to aspiration risk.
* Do not administer within 2 hours of other medications.
* For topical use, apply sparingly to affected areas.
