# Liquid Paraffin
## Overview
Liquid paraffin is a mineral oil that acts as an emollient and laxative. It lubricates the intestinal tract, softening stool and facilitating bowel movements.
## Primary Indications
* Constipation
## Adult Dosing
* **Laxative:** 10-30 mL once daily, typically at bedtime. Dosing can be adjusted based on response. Maximum of 30 mL per day.
## Pediatric Dosing
* **Constipation:** Age 1-12 years: 5-10 mL once daily. Age > 12 years: Use adult dosing.
* Dosing should be individualized and based on clinical response.
## Dose Adjustments
No dose adjustments are typically required for renal or hepatic impairment.
## Contraindications
* Appendicitis or symptoms of acute abdominal conditions.
* Intestinal obstruction or impaction.
* Hypersensitivity to liquid paraffin.
* Use with caution in patients who have difficulty swallowing due to the risk of lipid pneumonia.
## Adverse Effects
* Anal leakage, abdominal cramping, diarrhea, nausea, vomiting.
* Long-term or excessive use can lead to vitamin malabsorption (fat-soluble vitamins A, D, E, K) and potential dependence.
* Risk of lipid pneumonia if aspirated.
## Key Drug Interactions
* **Mineral Oil:** Can decrease the absorption of fat-soluble vitamins (A, D, E, K).
* **Cholestyramine:** May decrease the absorption of mineral oil.
* **Other Medications:** May reduce the absorption of other orally administered drugs due to the formation of an oily barrier. Administer other medications at least 2 hours before or after liquid paraffin.
## Monitoring
* Bowel movement frequency and consistency.
* Signs of abdominal distension or pain.
* For long-term use, consider monitoring for vitamin deficiencies.
## Clinical Pearls
* Administer at bedtime to minimize daytime disruption and anal leakage.
* Due to the risk of lipid pneumonia, avoid use in patients with a risk of aspiration (e.g., elderly, debilitated, those with impaired swallowing reflexes).
* Do not use for prolonged periods without medical advice.
* Ensure adequate fluid intake when using laxatives.
