# Liquid Paraffin
## Overview
Liquid paraffin is a mineral oil used as a laxative. It softens the stool by lubricating the intestinal lining, preventing water absorption from the stool.
## Primary Indications
Constipation.
## Adult Dosing
10-30 mL as a single dose, taken orally, usually at bedtime.
Maximum once-daily dose: 30 mL.
## Pediatric Dosing
* **Ages 1-5 years:** 5 mL once daily.
* **Ages 6-12 years:** 10 mL once daily.
* **Over 12 years:** 10-20 mL once daily.
Dosing for children under 1 year should only be under strict medical supervision, generally not recommended.
## Dose Adjustments
No specific dose adjustments are typically needed for hepatic or renal impairment.
## Contraindications
* Abdominal pain of unknown origin.
* Nausea or vomiting.
* Intestinal obstruction.
* Fecal impaction.
* Appendicitis.
* Rectal bleeding.
* Difficulty swallowing (risk of aspiration).
## Adverse Effects
The most common adverse effects include abdominal cramping, nausea, vomiting, diarrhea, and anal leakage. Long-term use can lead to vitamin malabsorption (fat-soluble vitamins A, D, E, K), particularly if taken with meals. Aspiration can cause lipoid pneumonia.
## Key Drug Interactions
* **Cholestyramine:** May decrease the absorption of liquid paraffin. Administer them at least 2 hours apart.
* **Fat-soluble vitamins (A, D, E, K):** Absorption may be reduced with chronic use.
* **Other laxatives:** Concomitant use increases the risk of adverse effects, particularly electrolyte imbalance.
## Monitoring
Monitor for effectiveness (bowel movements) and adverse effects (abdominal pain, cramping, anal leakage, signs of vitamin deficiency with chronic use).
## Clinical Pearls
* Administer between meals or at bedtime to minimize nutrient absorption interference.
* Ensure adequate fluid intake when using laxatives.
* Avoid use in patients with dysphagia due to the risk of aspiration and lipoid pneumonia.
* Consider alternatives for long-term management of constipation.
