## Liquid Paraffin + Milk of Magnesia + Sodium Pico Sulphate
### Overview
This is a combination product used for the relief of constipation. Liquid paraffin acts as a lubricant and stool softener. Milk of magnesia is an osmotic laxative. Sodium pico sulphate is a stimulant laxative.
### Primary Indications
* Short-term treatment of occasional constipation.
### Adult Dosing
* Standard dose: 15 mL orally once daily, usually at bedtime.
* Maximum dose: Typically 30 mL orally once daily, but consult specific product labeling.
### Pediatric Dosing
* **Ages 6-12 years:** 7.5 mL orally once daily, usually at bedtime.
* **Ages under 6 years:** Not recommended due to increased risk of adverse effects, particularly liquid paraffin aspiration. Consult a pediatrician for appropriate management.
### Dose Adjustments
* No specific dose adjustments are typically required for hepatic or renal impairment, but caution is advised.
### Contraindications
* Ileus, intestinal obstruction, or acute abdominal conditions (e.g., appendicitis, inflammatory bowel disease).
* Undiagnosed abdominal pain.
* Severe dehydration.
* Hypersensitivity to any component of the preparation.
* Use of liquid paraffin in patients with dysphagia, gastroesophageal reflux, or who are at risk of aspiration.
### Adverse Effects
* **Common:** Abdominal cramping, bloating, nausea, diarrhea.
* **Liquid Paraffin Specific:** Rectal leakage, anal irritation, lipid pneumonia (if aspirated).
* **Milk of Magnesia Specific:** Electrolyte disturbances (especially magnesium, in patients with renal impairment), hypermagnesemia.
* **Sodium Pico Sulphate Specific:** Nausea, vomiting, abdominal pain.
### Key Drug Interactions
* **Other Laxatives:** Increased risk of dehydration and electrolyte imbalance.
* **Milk of Magnesia:** May decrease absorption of other drugs by altering gastric pH or binding to them. Separate administration by at least 2 hours.
* **Mineral Oil:** Concurrent use increases the risk of adverse effects with liquid paraffin.
### Monitoring
* Monitor for signs of dehydration and electrolyte imbalance, especially in the elderly or those with renal impairment.
* Assess for return of bowel function.
### Clinical Pearls
* This product is intended for short-term use only (typically no more than 7 days). Prolonged use can lead to dependence.
* Ensure adequate fluid intake when using laxatives.
* Liquid paraffin should be administered with caution in elderly or debilitated patients due to the risk of aspiration and subsequent lipid pneumonia. Administration at bedtime or in a recumbent position increases this risk.
* Consider underlying causes of constipation before initiating treatment.
