# Liquid Paraffin + Milk of Magnesia + Sodium Picosulphate
## Overview
This combination product is used for the short-term relief of occasional constipation. Liquid paraffin acts as stool lubricant, milk of magnesia is an osmotic laxative, and sodium picosulphate is a stimulant laxative.
## Primary Indications
* Short-term treatment of occasional constipation.
## Adult Dosing
* **General:** Dosing is typically based on individual product formulations present in the combined preparation. The following are general guidelines for the individual components, which may be present in varying concentrations.
* **Liquid Paraffin:** 10-30 mL once daily, usually at bedtime.
* **Milk of Magnesia:** 15-60 mL once daily.
* **Sodium Picosulphate:** 5-15 mL once daily, usually at bedtime.
## Pediatric Dosing
* **General:** Safety and efficacy in children are not well-established for all components or combinations. Dosing should be based on the specific product and age, with cautious initiation.
* **Liquid Paraffin:** Generally not recommended for infants. For children 1-12 years, doses may range from 5-15 mL once daily, under medical supervision.
* **Milk of Magnesia:** For children 6-12 years, 15-30 mL once daily. For children 1-6 years, 5-15 mL once daily, under medical supervision.
* **Sodium Picosulphate:** Generally not recommended for children under 12 years unless advised by a healthcare professional.
## Dose Adjustments
* No specific dose adjustments are typically required for renal or hepatic impairment for these agents, but caution is advised, especially in severe disease, due to potential electrolyte imbalances with chronic use.
## Contraindications
* Intestinal obstruction, appendicitis, inflammatory bowel disease (e.g., Crohn's disease, ulcerative colitis), severe dehydration, abdominal pain of unknown cause.
* Hypersensitivity to any component.
* Liquid paraffin: Use with extreme caution or avoid in young children, elderly, or those with impaired swallowing reflex due to risk of lipoid pneumonia.
## Adverse Effects
* **Common:** Abdominal cramps, diarrhea, nausea, vomiting, electrolyte imbalance (especially with prolonged use or overdose).
* **Liquid Paraffin:** Anal leakage, perianal irritation. Lipoid pneumonia (if aspirated).
* **Milk of Magnesia:** Hypermagnesemia (risk increased in renal impairment).
* **Sodium Picosulphate:** Hypokalemia with chronic use.
## Key Drug Interactions
* **Reduced absorption of:** Digoxin, other orally administered drugs (due to laxative effect and altered GI transit).
* **Electrolyte disturbances:** May potentiate effects of cardiac glycosides, diuretics, corticosteroids (due to hypokalemia risk from sodium picosulphate with chronic use).
* **Liquid Paraffin:** Can reduce absorption of fat-soluble vitamins (A, D, E, K) and may interfere with enterohepatic circulation of certain drugs.
## Monitoring
* Monitor for effectiveness and signs of dehydration or electrolyte imbalance (e.g., weakness, dizziness, irregular heartbeat).
* Assess for abdominal pain, bloating, or other gastrointestinal distress.
## Clinical Pearls
* This is a combination product; specific dosing will depend on the concentration of each active ingredient per unit volume of the specific formulation. Always refer to the product packaging or local protocols.
* For short-term use only (typically not exceeding 1 week) unless directed by a healthcare provider. Chronic use can lead to dependence and electrolyte disturbances.
* Ensure adequate fluid intake when using laxatives.
* Liquid paraffin should be taken with care to avoid aspiration. It should not be taken immediately before lying down.
