# Liquid Paraffin + Milk of Magnesia + Sodium Picosulphate
## Overview
This is a fixed-dose combination laxative. Liquid paraffin is a lubricant laxative, milk of magnesia is an osmotic laxative, and sodium picosulphate is a stimulant laxative. It is typically used for short-term relief of occasional constipation.
## Primary Indications
Short-term relief of occasional constipation.
## Adult Dosing
Dosing varies by product formulation. Common adult dosing:
* **Liquid Paraffin:** 10-30 mL once daily.
* **Milk of Magnesia:** 15-60 mL once daily.
* **Sodium Picosulphate:** 5-15 mL once daily.
Always refer to the specific product's labeling for exact ingredient concentrations and recommended dosing.
## Pediatric Dosing
Dosing is generally **not recommended** for children under a certain age (often 12 years) without medical advice. Specific pediatric dosing, if deemed appropriate by a prescriber, depends on the age and weight of the child and the specific product. Consult product labeling or a pediatrician.
## Dose Adjustments
No specific dose adjustments are typically required for renal or hepatic impairment, but caution is advised in patients with significant comorbidities, particularly those affecting fluid and electrolyte balance.
## Contraindications
* Ileus, intestinal obstruction, or conditions that predispose to obstruction.
* Appendicitis, acute inflammatory bowel disease (e.g., Crohn's disease, ulcerative colitis).
* Abdominal pain of unknown etiology.
* Severe dehydration.
* Hypersensitivity to any component of the formulation.
## Adverse Effects
* Abdominal cramping, bloating, diarrhea, nausea, vomiting.
* Electrolyte imbalances (with prolonged or excessive use), particularly hypokalemia.
* Anal irritation or leakage.
* Liquid paraffin: Can impair absorption of fat-soluble vitamins; risk of lipid pneumonia if aspirated.
## Key Drug Interactions
* **Other Laxatives:** Increased risk of dehydration and electrolyte imbalance.
* **Diuretics and Corticosteroids:** Increased risk of hypokalemia.
* **Cardiac Glycosides:** Risk of toxicity may be increased with hypokalemia.
* **Antibiotics:** May reduce the absorption or efficacy of orally administered antibiotics.
* **Tetralcycline antibiotics:** May form insoluble complexes, reducing absorption.
## Monitoring
* Assess for effectiveness of bowel movement.
* Monitor for signs of dehydration (e.g., decreased urine output, dry mouth, thirst).
* Monitor for signs of electrolyte imbalance (e.g., muscle weakness, irregular heartbeat), especially with prolonged use or in vulnerable populations.
## Clinical Pearls
* Use for the shortest duration necessary.
* Advise patients to maintain adequate fluid intake (2-3 liters per day) while using this product.
* Avoid in patients with dysphagia due to the risk of aspiration of liquid paraffin.
* If constipation persists for more than 7 days, the patient should seek medical advice.
