# Liquid Paraffin + Milk of Magnesia + Sodium Picosulfate
## Overview
This combination is a multi-modal osmotic and stimulant laxative preparation. It acts by lubrication (paraffin), osmotic draw of fluid into the bowel (magnesium hydroxide/milk of magnesia), and direct stimulation of colonic peristalsis (sodium picosulfate).
## Primary Indications
Short-term treatment of chronic or refractory constipation; bowel preparation for clinical procedures (per institutional protocol).
## Adult Dosing
Standard dosing for constipation is typically 15–30 mL taken at bedtime. Follow specific manufacturer or local clinical guidelines, as concentration/formulations of the combination vary significantly. Do not exceed 30 mL/24 hours unless directed by a physician.
## Pediatric Dosing
- **Children >12 years:** Use adult dosing (15–30 mL).
- **Children <12 years:** Not recommended without specialist pediatric supervision.
- *Note:* Liquid paraffin is generally avoided in young children due to the risk of aspiration and interference with fat-soluble vitamin absorption.
## Dose Adjustments
- **Renal Impairment:** Use with extreme caution. Magnesium accumulation can lead to hypermagnesemia in patients with moderate-to-severe renal impairment (CrCl <30 mL/min).

## Contraindications

## Adverse Effects
- **Common:** Abdominal cramping, flatulence, diarrhea, nausea.
- **Serious:** Hypermagnesemia (lethargy, muscle weakness, hypotension), hypokalemia (with chronic overuse), lipid pneumonia (if aspirated), and rectal irritation.
## Key Drug Interactions
- **Absorption:** Liquid paraffin may decrease the absorption of fat-soluble vitamins (A, D, E, K).
- **Chelation:** Magnesium salts can decrease the absorption of tetracyclines, fluoroquinolones, and bisphosphonates. Space doses by at least 2 hours.
- **Diuretics:** Potential for additive hypokalemia if used with loop or thiazide diuretics long-term.
## Monitoring
- Monitor bowel patterns and consistency.
- Assess for signs of electrolyte disturbance (dizziness, muscle cramps) if used frequently.
- Renal function in elderly or patients with pre-existing renal issues.
## Clinical Pearls
- **Lipid Pneumonia Warning:** Never administer in a supine position or to patients with swallowing difficulties.
- **Hydration:** Patients must increase water intake significantly to allow the osmotic components to function safely.
- **Short-term use:** This combination is intended for intermittent use. Chronic reliance may lead to "lazy bowel" syndrome (atony).
- **Timing:** Effects typically occur within 6–12 hours.
