# Lintor (Hypothetical Drug)
## Overview
Lintor is a synthetic, small-molecule selective kinase inhibitor hypothesized for the management of chronic inflammatory pathways. Its mechanism involves the targeted modulation of intracellular signaling proteins. As this is a hypothetical agent, all clinical data provided are modeled based on standard pharmacological profiles for class-specific kinase inhibitors.
## Primary Indications
* Management of moderate-to-severe refractory plaque psoriasis.
* Off-label investigation: Adjunctive therapy for rheumatoid arthritis resistant to traditional DMARDs.
## Adult Dosing
* **Standard dose:** 10 mg orally once daily.
* **Maximum dose:** 20 mg orally once daily.
* *Note: Dosing should be titrated based on clinical response and hepatic clearance status.*
## Pediatric Dosing
* **Age 12–17 years:** 5 mg orally once daily.
* **Age <12 years:** Safety and efficacy have not been established. Use is not recommended outside of structured clinical trials.
## Dose Adjustments
* **Renal Impairment:** No adjustment required for CrCl ≥30 mL/min. For CrCl <30 mL/min, reduce dose to 5 mg every 48 hours.
* **Hepatic Impairment:** Avoid use in Child-Pugh Class C. For Child-Pugh B, reduce starting dose to 5 mg daily.
* **Drug Interactions:** Dose reduction to 5 mg is required if co-administered with strong CYP3A4 inhibitors (e.g., ketoconazole, clarithromycin).
## Contraindications
* Known hypersensitivity to Lintor or its excipients.
* Active, serious systemic infections (e.g., tuberculosis, sepsis).
* Moderate-to-severe hepatic impairment (Child-Pugh C).
## Adverse Effects
* **Common:** Upper respiratory tract infections, headache, nausea, mild transaminase elevation.

## Key Drug Interactions

* **Immunosuppressants:** Avoid concurrent use with biological DMARDs or other potent immunosuppressants due to compounded infection risk.
* **Live Vaccines:** Avoid live vaccines during therapy due to hypothesized blunted response and potential for disseminated infection.
## Monitoring
* **Baseline:** Complete blood count (CBC), liver function tests (LFTs), and screening for latent tuberculosis (TB) and Hepatitis B/C.
* **Ongoing:** Monitor LFTs every 4 weeks for the first 3 months, then every 3 months thereafter. Monitor CBC periodically for neutropenia or lymphopenia.
## Clinical Pearls
* Lintor should be taken at the same time each day, with or without food.
* Patients should be counseled to report signs of infection (fever, chills, persistent cough) immediately.
* Due to the nature of kinase inhibition, clinicians must maintain a high index of suspicion for dermatological changes; annual full-body skin examinations are recommended.
