# IV Hydrocortisone for Acute Asthma
## Overview
Hydrocortisone is a short-acting systemic corticosteroid used when rapid anti-inflammatory action is required. In the setting of acute asthma exacerbations, it is typically reserved for patients unable to tolerate oral therapy (e.g., severe vomiting, inability to swallow, or intubated patients).
## Primary Indications
* Management of acute, severe asthma exacerbations unresponsive to initial intensive treatment.
* Acute asthma requiring parenteral administration when oral intake is compromised.
## Adult Dosing
* **Initial Dose:** 100 mg IV every 6 to 8 hours.
* **Duration:** Usually continued until the patient can transition to oral corticosteroids (e.g., prednisone) or until clinical resolution.
## Pediatric Dosing
* **Dose:** 2 mg/kg to 4 mg/kg IV per dose.
* **Frequency:** Every 6 hours.
* **Maximum:** 100 mg per dose.

## Monitoring
* **Efficacy:** Monitor respiratory rate, oxygen saturation, physical assessment (wheezing, accessory muscle use), and peak expiratory flow (PEF) or FEV1 if applicable.
* **Safety:** Monitor blood glucose levels (especially in patients with diabetes or those at high risk for hyperglycemia) and blood pressure. Assess for signs of GI distress.
## Clinical Pearls
* **Preference for Oral:** Systemic corticosteroids for acute asthma are equally effective via the oral route (bioavailability is excellent) and should be preferred unless IV access is necessary for other reasons.
* **No "Steroid-Sparing" Benefit:** Intravenous steroids do not provide a faster clinical onset or superior outcomes compared to equivalent doses of oral prednisone/prednisolone.
* **Protocol Variation:** Always verify dose and interval against local institutional asthma management protocols, as clinical pathways may prioritize specific corticosteroid agents (e.g., methylprednisolone).
