# IV Hydrocortisone for Acute Asthma
## Overview
Hydrocortisone is a short-acting synthetic glucocorticoid with both glucocorticoid and significant mineralocorticoid activity. It is utilized in severe, acute asthma exacerbations when oral administration is not feasible (e.g., patient intolerance, severe vomiting, or impending respiratory failure).
## Primary Indications
* Status asthmaticus or severe acute asthma exacerbations unresponsive to initial intensive treatment.
## Adult Dosing
* **Loading Dose:** 100–250 mg IV push.
* **Maintenance:** 50–100 mg IV every 6 hours.

* **Mineralocorticoid Activity:** Hydrocortisone has higher mineralocorticoid activity compared to methylprednisolone or dexamethasone; consider this if fluid retention or electrolyte imbalance is a clinical concern.
* **Switching:** Oral route is preferred whenever stable; systemic bioavailability of oral prednisone is near 100%, making IV-to-oral conversion usually 1:1.
