# IV Hydrocortisone for Acute Asthma
## Overview
Hydrocortisone is a short-acting systemic corticosteroid used in acute asthma exacerbations when patients cannot tolerate oral medications or require rapid, potent anti-inflammatory effects. It acts by suppressing airway inflammation, reducing mucus production, and upregulating beta-2 receptors.
## Primary Indications
* Status asthmaticus or severe acute asthma exacerbations unresponsive to initial intensive bronchodilator therapy and oral corticosteroids.
* Patients with poor GI tolerance or vomiting who require systemic steroids.
## Adult Dosing
* **Initial Dose:** 100 mg IV every 6 to 8 hours.
* **Maximum Dose:** Generally 400 mg/day depending on clinical response, though dosing varies based on institution-specific protocols for severe respiratory distress.
## Pediatric Dosing
* **Standard Dose:** 5–10 mg/kg/day divided every 6 hours.

## Clinical Pearls
