# IV Hydrocortisone for Acute Asthma
## Overview
Hydrocortisone is a short-acting synthetic corticosteroid used in acute asthma exacerbations when oral administration is not feasible or the patient is in severe respiratory distress necessitating parenteral therapy. It acts by reducing airway inflammation and modulating the immune response.
## Primary Indications
- Management of acute, severe asthma exacerbations unresponsive to initial aggressive inhaled beta-agonist and anticholinergic therapy.
- Patients unable to tolerate oral corticosteroids (e.g., severe vomiting, intubated patients).
## Adult Dosing
- **Standard Dose:** 100 mg IV every 6 to 8 hours.
- **Duration:** Typically continued until the patient can transition to oral therapy (e.g., prednisone or prednisolone) or until symptoms resolve and the patient is stable for discharge.
## Pediatric Dosing
- **Dose:** 2 to 4 mg/kg IV per dose.
- **Frequency:** Every 6 hours.
- **Maximum:** 100 mg per dose.

## Monitoring
- **Clinical:** Respiratory rate, oxygen saturation, work of breathing, mental status.
- **Laboratory:** Serum glucose (especially in patients with diabetes), serum potassium (monitor for hypokalemia).
- **Adverse Effects:** Blood pressure, signs of fluid overload, daily weights.
## Clinical Pearls
- **Transitioning:** Transition to oral corticosteroids as soon as the patient can tolerate them. Prolonged tapering is generally not required for short-term courses (less than 7–10 days).
- **Efficacy:** No clear clinical advantage over oral prednisone exists for systemic bioavailability; use IV only when oral route is unavailable.
