# Iron Sucrose
## Overview
Iron sucrose is an intravenous iron preparation used to treat iron deficiency anemia. It replaces iron stores and allows for the production of hemoglobin.
## Primary Indications
* Iron deficiency anemia (IDA) in patients who are unable to tolerate or fail oral iron therapy.
* IDA in patients with chronic kidney disease (CKD), including those on hemodialysis and non-dialysis.
## Adult Dosing
Dosage is highly individualized based on the degree of iron deficiency and patient weight. Total dose required is often calculated using formulas.
* **Repletion Dose (Total Calculated Dose):** Typically ranges from 1000 mg to 2000 mg per treatment course.
* **Dosing Regimen:**
* Administered in multiple doses. A common regimen is 15 mL (750 mg) of iron sucrose (typically supplied as 100 mg/5 mL vials) administered intravenously over at least 10-15 minutes once or twice weekly until the total calculated dose is reached.
* Alternatively, a single infusion of 1000 mg (20 mL) may be given over at least 10 minutes, repeated as necessary to reach the total dose.
* **Maximum Single Infusion Dose:** 125 mg elemental iron (2.5 mL of a 100 mg/5 mL solution) for patients undergoing hemodialysis. For non-dialysis patients, single doses up to 1000 mg (20 mL) can be administered.
* **Maximum Cumulative Dose:** Up to 1000 mg elemental iron per dose for non-dialysis patients; up to 125 mg elemental iron for hemodialysis patients. The total iron required depends on the desired hemoglobin level and body weight.
## Pediatric Dosing
Dosing needs to be calculated based on weight and severity of anemia. Specific protocols may vary.
* **Repletion Dose:** Calculated based on weight:
* Children < 15 kg: 15 mg/kg of elemental iron.
* Children ≥ 15 kg: 1000 mg of elemental iron.
