# Icosapent Ethyl ## Overview Icosapent ethyl is a highly purified ethyl ester of eicosapentaenoic acid (EPA), an omega-3 fatty acid. Unlike dietary fish oil supplements containing DHA, icosapent ethyl does not increase LDL-C and is specifically indicated for cardiovascular risk reduction. ## Primary Indications * **Cardiovascular Risk Reduction:** Adjunct to maximally tolerated statin therapy in patients with elevated triglycerides (≥150 mg/dL) and established cardiovascular disease or diabetes with ≥2 additional risk factors. * **Severe Hypertriglyceridemia:** Adjunct to diet for reduction of triglyceride levels in adults with severe hypertriglyceridemia (≥500 mg/dL). ## Adult Dosing * **Cardiovascular Risk Reduction:** 2 grams orally twice daily (4 grams total daily). * **Severe Hypertriglyceridemia:** 2 grams orally twice daily (4 grams total daily). * **Administration:** Must be taken with food. Capsules should be swallowed whole; do not crush, dissolve, or chew. ## Pediatric Dosing Safety and efficacy have not been established in pediatric patients. Use is generally not recommended. ## Dose Adjustments * **Renal Impairment:** No dosage adjustment required. * **Hepatic Impairment:** No specific guidelines; use caution in patients with hepatic impairment due to potential for increased monitoring of liver enzymes. ## Contraindications * Known hypersensitivity (e.g., anaphylactic reaction) to icosapent ethyl or any component of the formulation. ## Adverse Effects * **Common:** Musculoskeletal pain (arthritis, back pain), peripheral edema, constipation, gout, atrial fibrillation or flutter. * **Serious:** Increased risk of atrial fibrillation/flutter requiring hospitalization; increased bleeding risk (rare, but caution in patients on anticoagulants or antiplatelet agents). ## Key Drug Interactions * **Anticoagulants/Antiplatelet Agents:** May increase the risk of bleeding. Monitor for signs of bleeding in patients on concomitant warfarin, clopidogrel, or other blood thinners. ## Monitoring * **Baseline/Routine:** Triglycerides, LDL-C, and ALT/AST. * **Clinical:** Monitor for symptoms of atrial fibrillation/flutter (palpitations, dizziness, dyspnea) and signs of abnormal bleeding. ## Clinical Pearls * **EPA vs. DHA:** Icosapent ethyl is pure EPA.
