# Hyoscine butylbromide (Buscopan)
## Overview
Hyoscine butylbromide is an anticholinergic medication. It acts as a smooth muscle relaxant, primarily affecting the gastrointestinal and genitourinary tracts.
## Primary Indications
* Symptomatic relief of abdominal cramps and spasms.
* Spasms of the urinary tract.
## Adult Dosing
* **Oral:** 10 mg to 20 mg up to 3-4 times daily.
* **Injection (IM/IV):** 20 mg every 8 hours as needed.
## Pediatric Dosing
Dosing in children is less well-established and should be guided by clinical judgment and local protocols.
* **Oral:** Some sources suggest 5 mg to 10 mg up to 3 times daily for children aged 6 to 12 years. Dosing below 6 years is generally not recommended without specific guidance.
* **Injection:** Generally not recommended for children.
## Dose Adjustments
No specific dose adjustments are typically required for renal or hepatic impairment. However, caution is advised in elderly patients due to potential anticholinergic side effects.
## Contraindications
* Hypersensitivity to hyoscine butylbromide or any component of the formulation.
* Myasthenia gravis.
* Untreated narrow-angle glaucoma.
* Tachyarrhythmias.
* Prostatic hypertrophy with urinary retention.
* Ileus or toxic megacolon.
* Pyloric stenosis and atonic bowels.
## Adverse Effects
Common anticholinergic effects:
* Dry mouth
* Blurred vision
* Tachycardia
* Urinary retention
* Constipation
Less common:
* Dizziness
* Nausea
* Skin reactions (e.g., rash, urticaria)
* Anaphylactic reactions (rare)
## Key Drug Interactions
* **Anticholinergics (e.g., tricyclic antidepressants, antihistamines):** Increased risk of anticholinergic side effects.
* **Dopamine antagonists (e.g., metoclopramide):** May reduce the efficacy of both drugs.
* **Beta-agonists:** May potentiate tachycardia.
## Monitoring
* Monitor for relief of symptoms.
* Observe for adverse anticholinergic effects, especially in the elderly or those with pre-existing conditions.
## Clinical Pearls
* Hyoscine butylbromide is a peripheral anticholinergic and generally does not cross the blood-brain barrier as readily as scopolamine, leading to fewer central nervous system side effects.
* It is useful for symptomatic relief of colic and spasms, but does not treat the underlying cause.
* The injectable formulation provides more rapid relief for acute, severe spasms.
