# Hyoscine Butylbromide (Buscopan)
## Overview
Hyoscine butylbromide is an anticholinergic medication that acts peripherally to block muscarinic receptors, primarily in the gastrointestinal and genitourinary tracts. It is thought to relieve smooth muscle spasms.
## Primary Indications
* Relief of abdominal cramping and pain associated with gastrointestinal spasms, irritable bowel syndrome (IBS), diverticulitis, and spastic constipation.
* Pre-procedural bowel preparation for imaging.
## Adult Dosing
* **Oral:** 10-20 mg three to five times daily. Maximum daily dose is typically 100 mg.
* **Intramuscular/Intravenous:** 20 mg every 6-8 hours as needed for acute spasms. This route is typically reserved for more severe, acute situations and should be administered by a healthcare professional.
## Pediatric Dosing
Dosing in pediatric populations is not well-established and should be determined by a physician, often based on weight. Limited data suggests:
* **Children 6-12 years:** 10 mg two to three times daily.
* **Infants and children under 6 years:** Use is generally discouraged or requires extreme caution and physician discretion.
## Dose Adjustments
No specific dose adjustments are typically required for renal or hepatic impairment, as the drug has limited systemic absorption and is poorly metabolized.
## Contraindications
* Hypersensitivity to hyoscine butylbromide or any component of the formulation.
* Angle-closure glaucoma.
* Myasthenia gravis.
* Ileus or intestinal obstruction.
* Tachyarrhythmias.
* Prostatic hypertrophy with urinary retention.
* Adynamic ileus.
* Megacolon.
## Adverse Effects
Common anticholinergic effects may occur, particularly at higher doses or with parenteral administration:
* Dry mouth
* Blurred vision
* Tachycardia
* Constipation
* Urinary retention
* Drowsiness
* Dizziness
Less common:
* Skin rash or urticaria
* Anaphylaxis (rare)
* Increased intraocular pressure
## Key Drug Interactions
* **Other Anticholinergics:** Additive anticholinergic effects.
* **Dopamine antagonists (e.g., metoclopramide):** May reduce the efficacy of both agents.
* **Antacids:** May decrease absorption if taken concurrently. Separate administration by at least 1 hour.
* **Beta-agonists:** May potentiate tachycardia.
## Monitoring
* Monitor for relief of symptoms.
* Monitor for anticholinergic adverse effects (dry mouth, blurred vision, constipation, urinary retention, tachycardia).
* Monitor for signs of glaucoma exacerbation if a patient has pre-existing risk factors.
## Clinical Pearls
* Hyoscine butylbromide has minimal central nervous system effects due to poor blood-brain barrier penetration.
* Oral formulations have variable bioavailability.
* Parenteral administration provides faster relief for acute, severe colicky pain.
* Ensure patients are not experiencing signs of bowel obstruction before initiating treatment, as the drug can worsen this condition.
