# Hyoscine butylbromide (Buscopan)
## Overview
Hyoscine butylbromide is an anticholinergic medication that acts peripherally to relax smooth muscle. It is primarily used for its antispasmodic effects.
## Primary Indications
* Abdominal pain and cramping associated with gastrointestinal (GI) disorders, such as irritable bowel syndrome (IBS).
* Biliary colic.
* Renal colic.
* Pre-operative medication to reduce secretions and prevent bradycardia.
## Adult Dosing
* **Oral:** 10-20 mg 3-4 times daily. Maximum daily dose is typically 100 mg.
* **Intramuscular/Intravenous:** 20 mg every 4-6 hours as needed. Maximum daily dose is typically 100 mg. Dosing may be more frequent in specific acute settings, consult local protocol.
## Pediatric Dosing
* **Oral:** 5-10 mg (or 0.1 mg/kg) 3 times daily in children aged 6-12 years. Dosing for younger children is generally not recommended and should be based on specific clinical assessment and local guidelines. Max dose 30mg/day.
* **Intramuscular/Intravenous:** 10 mg once in children aged 6-12 years. Dosing for younger children is generally not recommended.
## Dose Adjustments
No specific dose adjustments are typically required for hepatic or renal impairment, although caution is advised due to potential accumulation in severe disease.
## Contraindications
* Hypersensitivity to hyoscine butylbromide or any other component of the formulation.
* Narrow-angle glaucoma.
* Myasthenia gravis.
* Megalocolon.
* Ileus, intestinal atony, or obstructive GI conditions.
* Tachyarrhythmias.
* Prostatic hypertrophy with urinary retention.
* Pyloric stenosis and paralytic ileus.
## Adverse Effects
Common side effects include dry mouth, blurred vision, dizziness, drowsiness, tachycardia, sweating, and urinary retention. Less common include dysphagia, constipation, and skin reactions.
## Key Drug Interactions
* **Other anticholinergics (e.g., tricyclic antidepressants, antihistamines, antiparkinsonian drugs):** Increased risk of anticholinergic side effects.
* **Dopamine antagonists (e.g., metoclopramide):** May reduce the efficacy of both drugs.
* **Beta-blockers:** May mask tachycardia produced by hyoscine.
## Monitoring
* Monitor for efficacy in pain relief.
* Monitor for anticholinergic side effects, especially in the elderly.
* Monitor urinary output in patients at risk of retention.
## Clinical Pearls
* Hyoscine butylbromide is a quaternary ammonium compound, limiting its passage across the blood-brain barrier, thus having fewer central anticholinergic effects compared to other anticholinergics like atropine.
* IV administration provides rapid onset of action for acute pain.
* Effectiveness can vary, and it may not be suitable for all types of abdominal pain.
