# Hyoscine butylbromide (Buscopan)
## Overview
Hyoscine butylbromide is an anticholinergic, antispasmodic medication that primarily acts on the smooth muscle of the gastrointestinal and genitourinary tracts. It competitively inhibits acetylcholine at muscarinic receptors, leading to relaxation of smooth muscle.
## Primary Indications
* Symptomatic relief of abdominal cramps and pain associated with gastrointestinal disorders (e.g., irritable bowel syndrome, diverticulitis).
* Relief of spasms of the biliary and urinary tracts.
## Adult Dosing
* **Oral:** 20 mg four times daily. Maximum dose: Not explicitly established for chronic use, but higher doses may increase anticholinergic side effects.
* **Intramuscular/Intravenous:** 20 mg every 6-8 hours as needed. Maximum dose: 100 mg per 24 hours.
## Pediatric Dosing
* **Oral:**
* Children 6-12 years: 10 mg three times daily.
* Children under 6 years: Safety and efficacy not established. Dosing is not recommended.
* **Intramuscular/Intravenous:**
* Children 0-12 years: 0.3-0.6 mg/kg per dose, administered every 6-8 hours as needed. Maximum dose: Not explicitly established for IV/IM use, but follow adult maximums cautiously.
## Dose Adjustments
No specific dose adjustments are typically required for hepatic or renal impairment. However, caution is advised in elderly patients due to potential for increased sensitivity to anticholinergic effects.
## Contraindications
* Hypersensitivity to hyoscine butylbromide or other hyoscyamine derivatives.
* Myasthenia gravis.
* Glaucoma (angle-closure glaucoma).
* Prostatic hypertrophy with urinary retention.
* Tachyarrhythmias.
* Ileus or paralytic ileus.
* Gastrointestinal obstruction or stenosis.
* Megacolon.
## Adverse Effects
Common anticholinergic effects: dry mouth, blurred vision, constipation, urinary retention, tachycardia, drowsiness, dizziness. Less common: allergic reactions, difficulty with accommodation.
## Key Drug Interactions
* **Anticholinergic agents (e.g., tricyclic antidepressants, antihistamines, phenothiazines):** Increased risk of anticholinergic side effects.
* **Dopamine antagonists (e.g., metoclopramide):** May reduce the effects of both drugs.
* **Beta-blockers:** May increase the risk of tachycardia.
## Monitoring
Monitor for relief of abdominal pain/cramps and for signs/symptoms of anticholinergic side effects (e.g., dry mouth, blurred vision, urinary hesitancy, constipation, increased heart rate).
## Clinical Pearls
* Hyoscine butylbromide is a peripheral anticholinergic and has minimal central nervous system effects at standard doses, unlike some other anticholinergic agents.
* Its effectiveness for severe pain or spasms may be limited.
* Ensure adequate hydration, especially if constipation is a concern.
