# Hyoscine butylbromide (Buscopan)
## Overview
Hyoscine butylbromide is an anticholinergic antispasmodic medication. It works by blocking muscarinic receptors in the smooth muscle of the gastrointestinal tract, reducing spasms and motility.
## Primary Indications
* Symptomatic relief of painful spasms of the stomach and intestines.
* Relief of spasms of the biliary tract.
* Relief of spasms of the genitourinary tract.
## Adult Dosing
* **Oral:** 10-20 mg three times daily. Maximum daily dose is typically 100 mg.
* **Intramuscular/Intravenous:** 20 mg every 6-8 hours as needed. Maximum daily dose for parenteral administration is typically 100 mg.
Dosing for specific procedures (e.g., endoscopy) may vary based on local protocol.
## Pediatric Dosing
* **Oral:**
* 1-12 years: 5-10 mg three times daily.
* **Intramuscular/Intravenous:**
* Data is limited for routine use in children. Caution is advised. Some sources suggest doses similar to adults may be used cautiously in older children for intractable spasms.
## Dose Adjustments
No dose adjustment is typically required for renal or hepatic impairment, though caution is advised in severe cases due to potential accumulation of anticholinergic effects.
## Contraindications
* Hypersensitivity to hyoscine butylbromide or any component of the formulation.
* Myasthenia gravis.
* Gastrointestinal obstruction or ileus.
* Tachyarrhythmias.
* Angle-closure glaucoma.
* Prostatic hypertrophy with urinary retention.
* Megacolon.
* Gastrointestinal bleeding.
## Adverse Effects
Common adverse effects are related to anticholinergic activity and include dry mouth, blurred vision, tachycardia, constipation, and urinary retention. Less common effects include skin reactions and dizziness.
## Key Drug Interactions
* **Anticholinergic agents:** Additive anticholinergic effects (e.g., tricyclic antidepressants, antihistamines, antiparkinsonian drugs).
* **Dopamine antagonists:** May reduce the efficacy of prokinetic drugs like metoclopramide.
* **Beta-adrenergic agonists:** May increase the risk of tachycardia.
## Monitoring
Monitor for relief of symptoms and for adverse effects, particularly dry mouth, blurred vision, urinary retention, and tachycardia.
## Clinical Pearls
* Hyoscine butylbromide is a quaternary ammonium compound, which limits its ability to cross the blood-brain barrier, resulting in fewer central nervous system side effects compared to other anticholinergics.
* It is primarily used for peripheral smooth muscle spasm.
* Parenteral administration offers rapid onset of action for severe colicky pain.
