# Hyoscine butylbromide (Buscopan)
## Overview
Hyoscine butylbromide is an anticholinergic medication that works by blocking muscarinic receptors, leading to smooth muscle relaxation. It is primarily used for its antispasmodic effects.
## Primary Indications
* Symptomatic relief of abdominal cramping and pain associated with gastrointestinal spasms, such as irritable bowel syndrome (IBS).
* Pre-procedural bowel preparation to reduce fecal burden and improve visualization in diagnostic procedures like colonoscopy.
## Adult Dosing
* **Oral:** 10-20 mg three times daily. Maximum daily dose: 100 mg.
* **Intramuscular/Intravenous:** 20 mg every 6-8 hours as needed. Typically administered by healthcare professionals. Maximum daily dose: 100 mg.
## Pediatric Dosing
Dosing in children is less established and should be guided by clinical judgment and available product information, often at lower doses than adults.
* **Oral:** Dosing varies by age and weight. For children over 6 years: 10 mg two to three times daily. Younger children's dosing should be individualized, often starting at 5 mg.
* **Intramuscular/Intravenous:** Use in children is generally not recommended unless under strict medical supervision and for specific indications.
## Dose Adjustments
No specific dose adjustments are routinely required for hepatic or renal impairment.
## Contraindications
* Hypersensitivity to hyoscine butylbromide or any excipients.
* Narrow-angle glaucoma.
* Myasthenia gravis.
* Tachyarrhythmias.
* Mega colon.
* Ileus or intestinal atony.
* Prostatic hypertrophy with urinary retention.
* Lactation.
## Adverse Effects
Common adverse effects include dry mouth, blurred vision, drowsiness, and urinary hesitancy. Less common effects include tachycardia, constipation, and allergic reactions.
## Key Drug Interactions
* **Other anticholinergics:** Additive anticholinergic effects (e.g., tricyclic antidepressants, antihistamines).
* **Dopamine antagonists:** May reduce the efficacy of prokinetic agents like metoclopramide.
* **Beta-agonists:** May increase heart rate.
## Monitoring
Monitor for therapeutic effectiveness and signs of anticholinergic side effects, especially in elderly patients or those with pre-existing conditions.
## Clinical Pearls
* Hyoscine butylbromide acts locally on the gut and has minimal systemic anticholinergic effects compared to other agents like hyoscine hydrobromide, making it a preferred option for GI spasms.
* IV administration can provide rapid relief for acute, severe spasms.
* Caution is advised in patients with glaucoma, tachycardia, or urinary retention.
