# Hyoscine butylbromide (Buscopan)
## Overview
Hyoscine butylbromide is an anticholinergic medication that acts as a smooth muscle relaxant, primarily affecting the gastrointestinal and genitourinary tracts.
## Primary Indications
* Treatment of abdominal pain and cramps associated with smooth muscle spasms, such as in irritable bowel syndrome (IBS).
* Pre-procedural bowel preparation to reduce intestinal motility.
* Management of biliary or renal colic.
## Adult Dosing
* **Oral:** 20 mg three times daily. Maximum daily dose is typically 100 mg.
* **Intramuscular/Intravenous:** 20 mg every 4-6 hours as needed. Maximum daily dose is typically 100 mg. IV administration is usually reserved for acute, severe spasms.
## Pediatric Dosing
* **Oral:** Dosing in children is less established. Generally, 5-10 mg every 8 hours for children 6-12 years may be considered, but caution is advised. Consult specific pediatric guidelines.
* **Intramuscular/Intravenous:** Not typically recommended for children under 12 years due to limited safety data.
## Dose Adjustments
No specific dose adjustments are typically required for hepatic or renal impairment, but caution should be exercised in patients with severe disease.
## Contraindications
* Hypersensitivity to hyoscine butylbromide or any excipients.
* Angle-closure glaucoma.
* Myasthenia gravis.
* Mega colon.
* Tachyarrhythmias.
* Ileus or intestinal obstruction.
* Urinary retention.
## Adverse Effects
Common adverse effects include dry mouth, blurred vision, constipation, and urinary retention. Less common effects include dizziness, drowsiness, tachycardia, and allergic reactions.
## Key Drug Interactions
* **Anticholinergics:** Additive anticholinergic effects can occur, increasing the risk of dry mouth, blurred vision, constipation, and urinary retention (e.g., tricyclic antidepressants, antihistamines, antiparkinsonian agents).
* **Dopamine antagonists:** May reduce the efficacy of prokinetic drugs like metoclopramide.
* **Beta-agonists:** May increase heart rate.
## Monitoring
Monitor for resolution of symptoms, signs of anticholinergic side effects (dry mouth, blurred vision, urinary retention, constipation), and allergic reactions.
## Clinical Pearls
* Hyoscine butylbromide primarily acts peripherally, leading to fewer central nervous system effects compared to other anticholinergics like atropine.
* It is often used for symptomatic relief of spasms rather than addressing the underlying cause.
* Ensure adequate hydration and stool softeners if constipation is a concern with prolonged use.
