# Hyoscine butylbromide (Buscopan)
## Overview
Hyoscine butylbromide is an anticholinergic medication that acts primarily on the smooth muscle of the gastrointestinal tract. It is a quaternary ammonium derivative of hyoscine, which limits its ability to cross the blood-brain barrier, resulting in fewer central nervous system effects compared to other anticholinergics.
## Primary Indications
* Cramps and spasms of the gastrointestinal tract
* Biliary colic
* Renal colic
## Adult Dosing
* **Oral:** 10-20 mg (1-2 tablets) three to five times daily. Maximum daily dose typically not specified but should be guided by symptom relief and tolerability.
* **Parenteral (IM/IV):** 20 mg every 4-6 hours as needed. Maximum daily dose typically 100 mg. IV administration is generally preferred for rapid relief if clinically indicated and tolerated.
## Pediatric Dosing
Dosing in children is less well-established and should be approached with caution.
* **Oral (age 6-12 years):** 10 mg two to three times daily.
* **Parenteral:** Data is limited. Local protocols and expert consultation are recommended if parenteral administration is considered in children.
## Dose Adjustments
No dose adjustments are typically required for hepatic or renal impairment due to minimal systemic absorption and rapid metabolism.
## Contraindications
* Hypersensitivity to hyoscine butylbromide or any component of the formulation.
* Myasthenia gravis.
* Paralytic ileus or intestinal obstruction.
* Tachyarrhythmias.
* Angle-closure glaucoma.
* Prostatic hypertrophy with urinary retention.
* Megacolon.
## Adverse Effects
Common adverse effects include dry mouth, constipation, blurred vision, tachycardia, urinary retention, and skin reactions (e.g., rash, urticaria). Less common but serious effects can include anaphylaxis.
## Key Drug Interactions
* **Amantadine:** Increased risk of anticholinergic adverse effects.
* **Antidepressants (tricyclic, MAOIs):** Enhanced anticholinergic effects.
* **Antihistamines:** Potentiated anticholinergic effects.
* **Dopamine antagonists (e.g., metoclopramide):** Reduced efficacy of both agents.
* **Other anticholinergics:** Additive anticholinergic effects.
## Monitoring
Monitor for efficacy in symptom relief. Observe for adverse effects, particularly urinary retention, constipation, and signs of glaucoma exacerbation if risk factors are present.
## Clinical Pearls
* Hyoscine butylbromide is effective for symptomatic relief of smooth muscle spasms.
* Its limited CNS penetration reduces the risk of drowsiness and other central effects commonly seen with older anticholinergics.
* Parenteral administration provides more rapid onset of action.
* Use with caution in patients with known risk factors for glaucoma, prostatic hypertrophy, or cardiac arrhythmias.
