# Hyoscine butylbromide
## Overview
Hyoscine butylbromide is an anticholinergic medication that acts as a peripheral antispasmodic. It works by blocking muscarinic acetylcholine receptors in the smooth muscle of the gastrointestinal tract, leading to relaxation and relief of spasms.
## Primary Indications
* Symptomatic relief of abdominal cramps and pain associated with gastrointestinal disorders.
* Preoperative medication to reduce secretions.
* Management of irritable bowel syndrome (IBS).
## Adult Dosing
* **Oral:** 10-20 mg three to five times daily. Maximum daily dose: 100 mg.
* **Intramuscular/Intravenous:** 20 mg every 6-8 hours as needed. Maximum daily dose: 100 mg.
## Pediatric Dosing
* **Oral:** 5-10 mg two to three times daily for children aged 6-12 years. Dosing for younger children is not well-established and should be based on clinical judgment.
* **Intramuscular/Intravenous:** 20 mg every 6-8 hours as needed for children aged 12 years and older. Dosing for younger children is not well-established and should be based on clinical judgment.
## Dose Adjustments
No specific dose adjustments are typically required for renal or hepatic impairment, but caution is advised in patients with severe dysfunction.
## Contraindications
* Hypersensitivity to hyoscine butylbromide or any component of the formulation.
* Glaucoma (especially narrow-angle glaucoma).
* Myasthenia gravis.
* Tachycardia.
* Obstructive gastrointestinal or urinary tract disorders.
* Prostatic hypertrophy with urinary retention.
* Ileus.
* Megacolon.
## Adverse Effects
Common adverse effects include dry mouth, blurred vision, drowsiness, dizziness, difficulty urinating, and constipation. Less common effects may include tachycardia and skin rash.
## Key Drug Interactions
* **Amantadine, quinidine, tricyclic antidepressants, MAOIs:** Increased anticholinergic effects.
* **Metoclopramide, domperidone:** May antagonize the gastrointestinal effects of hyoscine butylbromide.
* **Cisapride:** May decrease the efficacy of hyoscine butylbromide.
## Monitoring
* Assess for relief of abdominal pain and spasms.
* Monitor for adverse effects, particularly urinary retention, constipation, and signs of anticholinergic toxicity.
## Clinical Pearls
* Hyoscine butylbromide is a peripheral anticholinergic and is less likely to cause central nervous system side effects compared to centrally acting anticholinergics like atropine.
* For acute severe spasms, intravenous administration may provide more rapid relief.
* Ensure adequate hydration and fiber intake to mitigate constipation.
