# Hyoscine butylbromide (Buscopan)
## Overview
Hyoscine butylbromide is an antimuscarinic agent that acts peripherally on muscarinic receptors in the gastrointestinal tract. It reduces smooth muscle spasms and cramps.
## Primary Indications
* Symptomatic treatment of abdominal spasms and cramps.
* Treatment of irritable bowel syndrome (IBS) symptoms.
## Adult Dosing
* **Oral:** 20 mg four times daily.
* **Parenteral (Intramuscular/Intravenous):** 20 mg every 4-6 hours as needed. Maximum dose is typically 100 mg per day, but this may vary with local protocol.
## Pediatric Dosing
* **Oral:**
* Children 6-12 years: 10 mg three times daily.
* Children under 6 years: Dosage not established. Use with caution and under medical supervision.
* **Parenteral:** Dosing in children is not well-established and should be guided by specialist advice.
## Dose Adjustments
No specific dose adjustments are typically required for hepatic or renal impairment. However, caution is advised in elderly patients due to potential anticholinergic effects.
## Contraindications
* Hypersensitivity to hyoscine butylbromide or any component of the formulation.
* Myasthenia gravis.
* Glaucoma (especially angle-closure glaucoma).
* Tachyarrhythmias.
* Prostatic hypertrophy with urinary retention.
* Gastric or intestinal obstruction.
* Paralytic ileus.
* Megacolon.
## Adverse Effects
* **Common:** Dry mouth, blurred vision, constipation, urinary retention, tachycardia, palpitations.
* **Less Common:** Drowsiness, dizziness, skin rash, urticaria.
* **Rare:** Anaphylactic reactions, anaphylactic shock.
## Key Drug Interactions
* **Other Anticholinergics (e.g., tricyclic antidepressants, antihistamines, antipsychotics):** May potentiate anticholinergic side effects.
* **Dopamine antagonists (e.g., metoclopramide):** Reduced efficacy of both drugs.
* **Tachycardia-inducing agents (e.g., beta-agonists):** Increased risk of tachycardia.
## Monitoring
* Monitor for signs of anticholinergic effects (dry mouth, blurred vision, urinary retention, constipation, tachycardia).
* Observe for signs of hypersensitivity reactions, especially with parenteral administration.
## Clinical Pearls
* Hyoscine butylbromide has peripheral antispasmodic activity and minimal central nervous system effects compared to other anticholinergics.
* It is effective for symptomatic relief of cramps and spasms but does not treat the underlying cause.
* Parenteral administration offers faster onset of action for acute painful spasms.
