# L-Ornithine L-Aspartate (Hepamerz)
## Overview
L-Ornithine L-Aspartate (LOLA) is a combination of two amino acids, ornithine and aspartate, believed to aid in ammonia detoxification in the liver.
## Primary Indications
Treatment of hyperammonemia, particularly in patients with liver disease (e.g., hepatic encephalopathy).
## Adult Dosing
* **Oral:** Typically 3-6 grams three times daily, with doses up to 9 grams three times daily reported.
* **Intravenous (IV):** 5-10 grams daily, often infused over 30-60 minutes. Doses up to 20 grams daily in divided doses have been used in severe cases. Specific IV dosing protocols may vary; consult local guidelines.
## Pediatric Dosing
Dosing in children is not well-established. Data is limited, and use should be based on specialist recommendation and careful monitoring. Some sources suggest a starting dose of 100-150 mg/kg/day orally in divided doses for hepatic encephalopathy, but this is not universally adopted.
