# Hepamerz (l-Ornithine l-Aspartate)
## Overview
Hepamerz is a combination product containing l-ornithine and l-aspartate. It is used to treat hyperammonemia associated with liver insufficiency.
## Primary Indications
* Treatment of overt hepatic encephalopathy (HE) and subclinical hepatic encephalopathy.
* Management of hyperammonemia in patients with liver cirrhosis.
## Adult Dosing
* **Oral:** Typically, 3-6 grams three times daily, divided into three doses. The total daily dose can range from 9 grams to 18 grams.
* **Intravenous:** Typically, 5 grams or 10 grams infused over 30-60 minutes, once or twice daily. The decision on infusion rate and total daily dose often depends on severity of hyperammonemia and clinical status, and may be guided by local protocols.
## Pediatric Dosing
Dosing in pediatric patients is not well-established and should be based on clinical judgment, severity of disease, and local institutional protocols. Data is limited.
## Dose Adjustments
No specific dose adjustments are generally recommended for renal or hepatic impairment, beyond the underlying indication of liver disease itself.
## Contraindications
* Known hypersensitivity to l-ornithine or l-aspartate.
* Severe renal failure (creatinine clearance < 25 mL/min) may warrant caution due to potential accumulation of amino acids.
## Adverse Effects
Common adverse effects include nausea, vomiting, abdominal pain, and diarrhea, especially with oral administration. Less commonly, rash, itching, and muscle pain may occur.

* Oral administration should ideally be taken with meals or after meals to reduce gastrointestinal upset.
* Response to therapy can vary. It is not a substitute for addressing the underlying cause of liver disease and hyperammonemia.
