# Hepamerz (l-Ornithine l-Aspartate)
## Overview
L-Ornithine L-Aspartate (LOLA) is a combination of two amino acids, ornithine and aspartate. It is thought to stimulate the detoxification of ammonia in the liver and peripheral tissues and to improve energy metabolism.
## Primary Indications
* Supportive treatment of latent and manifest hepatic encephalopathy (HE) in patients with chronic liver disease.
## Adult Dosing
* **Oral:** Typically 3000 mg to 6000 mg (3 to 6 grams) three times daily.
* **Intravenous:** Typically 5 grams to 10 grams daily, administered as a slow infusion. The exact dosage and frequency may depend on the severity of hepatic encephalopathy and local protocol.
## Pediatric Dosing
* There is limited well-established pediatric dosing for L-Ornithine L-Aspartate. Dosing in pediatric patients should be based on clinical judgment and consultation with literature or specialized resources, and is often extrapolated from adult data or weight-based calculations.
## Dose Adjustments
* No specific dose adjustments are routinely recommended for renal or hepatic impairment, but caution should be exercised.
## Contraindications
* Severe renal impairment.
* Known hypersensitivity to ornithine, aspartate, or any component of the formulation.
* Rarely, patients with AV block may require caution due to potential electrolyte effects.
## Adverse Effects
* Gastrointestinal disturbances: nausea, vomiting, abdominal pain, diarrhea.
* Rarely: skin rash, headache, dizziness.
* With IV administration: local irritation at the injection site.
## Key Drug Interactions
* No significant drug interactions have been definitively established. However, monitor patients receiving other medications that affect ammonia levels or liver function.
## Monitoring
* Monitor for signs and symptoms of hepatic encephalopathy.
* Monitor electrolytes, especially in patients with renal impairment or at risk.
* Monitor liver function tests periodically.
## Clinical Pearls
* LOLA is often used as an adjunct to other therapies for hepatic encephalopathy.
* The oral formulation may be used for long-term management of latent HE, while IV formulations are typically used for more acute or severe presentations.
* It is crucial to ensure adequate hydration, especially with IV administration.
