# Galvusmet
## Overview
- **Classification**: Oral Antidiabetic (DPP-4 Inhibitor + Biguanide combination)
- **Mechanism**: Vildagliptin inhibits DPP-4, increasing incretin hormones, enhancing insulin release and suppressing glucagon. Metformin reduces hepatic glucose production, decreases intestinal glucose absorption, and improves insulin sensitivity.
## Primary Indications
1. **Type 2 Diabetes Mellitus** - As an adjunct to diet and exercise in adults.
2. **Combination Therapy** - When vildagliptin and metformin are appropriate, or when either monotherapy or metformin with another oral antidiabetic agent does not achieve adequate glycemic control.
## Adult Dosing
### Standard Dosing
**Type 2 Diabetes Mellitus**
- **Dose**: **50 mg vildagliptin / 500 mg metformin**, **50 mg / 850 mg**, or **50 mg / 1000 mg**.
- **Frequency**: Twice daily (BID)
- **Route**: Oral
- **Maximum Dose (daily)**: **100 mg vildagliptin** and **2000 mg metformin**
- **Special Considerations**: Take with or immediately after meals to reduce GI side effects of metformin.
### Dose Adjustments
- **Renal Impairment**:
- **CrCl ≥ 60 mL/min**: No dose adjustment needed.
- **CrCl 30-59 mL/min**: Max **50 mg vildagliptin / 1000 mg metformin** daily (e.g., **50 mg/500 mg** BID or **1000 mg** once daily). Metformin initiation not recommended if CrCl < 45 mL/min.
- **CrCl < 30 mL/min**: **Contraindicated**.
- **Hepatic Impairment**: **Contraindicated** for metformin component. Vildagliptin not recommended for moderate to severe impairment.
- **Elderly Patients**: Monitor renal function more frequently due to potential age-related decline.
## Pediatric Dosing
Galvusmet is **not indicated** for use in children and adolescents. Safety and efficacy have not been established in patients under 18 years of age.

## Safety Information
### Contraindications
- **Absolute**: Hypersensitivity to vildagliptin or metformin.
- **Absolute**: Renal impairment (eGFR < 30 mL/min/1.73m²).
- **Absolute**: Acute or chronic metabolic acidosis, including diabetic ketoacidosis (DKA).
- **Absolute**: Acute heart failure, unstable angina, or recent myocardial infarction.
- **Absolute**: Hepatic impairment.
- **Absolute**: Acute or chronic disease with tissue hypoxia (e.g., respiratory failure, shock).

### Common Adverse Effects
- **Very Common (>10%)**: Nausea, vomiting, diarrhea, abdominal pain, loss of appetite (metformin-related).
- **Common (1-10%)**: Headache, dizziness, tremor, asthenia, nasopharyngitis, arthralgia.
- **Serious but Rare**: Pancreatitis, Lactic Acidosis (metformin), Hepatotoxicity, Angioedema, Bullous Pemphigoid.
### Key Drug Interactions
- **Iodinated Contrast Media**: Hold metformin **before and 48 hours after** procedures involving contrast. Risk of lactic acidosis.
- **Cationic Drugs (e.g., Cimetidine, Furosemide, Nifedipine)**: May increase metformin concentration. Monitor for metformin toxicity.
- **Carbonic Anhydrase Inhibitors (e.g., Topiramate)**: Increased risk of lactic acidosis.
- **Alcohol**: Potentiates metformin's effect on lactate metabolism, increasing lactic acidosis risk. Avoid excessive intake.
## Monitoring & Follow-up
- **Before Treatment**:
- Renal function (eGFR).
- HbA1c.
- Liver function tests (ALT, AST).
- **During Treatment**:
- Renal function: At least **annually** if eGFR ≥ 60 mL/min; **2-4 times a year** if eGFR 30-59 mL/min.
- HbA1c: **Every 3-6 months**.
- Vitamin B12 levels: **Annually** in patients with risk factors for deficiency or on prolonged metformin therapy.
- Clinical Signs: Monitor for pancreatitis, hepatotoxicity, and symptoms of lactic acidosis.
- **Clinical Signs**: Watch for unexplained muscle pain, malaise, respiratory distress, increased somnolence (possible lactic acidosis).
## Clinical Pearls
- 💡 **Timing**: Always take Galvusmet **with or immediately after meals** to minimize GI side effects of metformin.
- 💡 **Lactic Acidosis Risk**: Counsel patients on the symptoms of lactic acidosis and to seek immediate medical attention if they occur. Inform them about situations increasing risk (e.g., severe illness, dehydration, alcohol).
- 💡 **Renal Function**: Emphasize the importance of regular renal function monitoring, especially in elderly patients or those with risk factors for kidney disease.
- 💡 **Missed Dose**: If a dose is missed, take the next dose at the regular scheduled time. Do not double the dose.
