# Folic Acid
## Overview
Folic acid (Vitamin B9) is a water-soluble B vitamin essential for DNA synthesis, repair, and erythropoiesis. It acts as a coenzyme in the synthesis of nucleic acids and the metabolism of amino acids.
## Primary Indications
* Treatment of megaloblastic and macrocytic anemias due to folate deficiency.
* Prevention of neural tube defects (NTDs) during pregnancy.
* Prophylaxis in chronic hemolytic states (e.g., sickle cell disease) or malabsorption syndromes.
* Reduction of hematologic/GI side effects associated with methotrexate therapy.
## Adult Dosing
* **Folate Deficiency:** 0.4–1 mg daily. Severe cases may require up to 5 mg daily.
* **Pregnancy (Prevention of NTDs):** 0.4–0.8 mg daily, starting at least 1 month before conception and continuing through the first trimester. High-risk patients (history of NTD pregnancy) may require 4–5 mg daily per perinatology guidelines.
* **Methotrexate Supplementation:** 1–5 mg daily (or 5 mg weekly), typically skipping the dose on the day of methotrexate administration.
## Pediatric Dosing
* **Deficiency (Infants):** 0.1 mg daily.
* **Deficiency (Children 1–11 years):** 0.4 mg daily.
* **Deficiency (>12 years):** Same as adult dosing.
* **Chronic Hemolytic Anemia/Malabsorption:** 0.1–1 mg daily depending on age and severity.
## Dose Adjustments
* **Renal/Hepatic Impairment:** No specific dosage adjustments established; use caution in patients with severe impairment.
* Note: Dosing often depends on local institutional protocols, especially for methotrexate rescue or specific hematologic conditions.
## Contraindications
* Hypersensitivity to folic acid or any component of the formulation.
* **Warning:** Do not use as monotherapy for pernicious anemia or other vitamin B12 deficiency-related anemias; folic acid may mask neurological progression while partially correcting hematologic indices.
## Adverse Effects
* Generally well-tolerated.
* Rare: Allergic reactions (rash, pruritus, bronchospasm), gastrointestinal upset, altered sleep patterns, or irritability at very high doses.
## Key Drug Interactions
* **Methotrexate/Pemetrexed:** Folic acid may decrease the efficacy of these folate-antagonist chemotherapy agents.
* **Anticonvulsants (Phenytoin, Carbamazepine, Valproate):** May decrease serum concentrations of these drugs and increase the risk of seizures.
* **Sulfonamides:** May inhibit folic acid absorption.
## Monitoring
* Monitor hemoglobin, hematocrit, and RBC indices for resolution of macrocytic anemia.
* Monitor folate levels if deficiency is suspected or for long-term replenishment.
* Ensure vitamin B12 status is confirmed before initiating high-dose folic acid to avoid masking subacute combined degeneration of the spinal cord.
## Clinical Pearls
* Folic acid is essential for rapidly dividing cells; therefore, deficiency leads primarily to hematologic effects.
* For patients starting methotrexate, initiate folic acid 24 hours after MTX dose or maintain consistent daily low-dose supplementation to minimize toxicity without compromising oncologic efficacy.
* In the presence of vitamin B12 deficiency, folic acid will correct the anemia but will *not* prevent or reverse neurologic damage.
