# Folic Acid
## Overview
Folic acid (vitamin B9) is a water-soluble B vitamin essential for DNA synthesis, repair, and erythropoiesis. Exogenous supplementation is required to prevent or treat deficiency states.
## Primary Indications
* Treatment of folate deficiency anemia.
* Prophylaxis of neural tube defects (NTDs) in pregnancy and pre-conception.
* Prevention of megaloblastic anemia secondary to drugs (e.g., methotrexate, sulfasalazine).
* Prophylaxis in chronic hemolytic states (e.g., sickle cell disease).
## Adult Dosing
* **Folate Deficiency:** 1 mg daily or up to 5 mg daily depending on the severity of malabsorption.
* **Pregnancy/Pre-conception (Standard):** 400 mcg to 800 mcg daily.
* **High-Risk Pregnancy (e.g., history of NTD, anti-epileptic use):** 4 mg to 5 mg daily (start at least 1 month before conception through the first trimester).
* **Prevention of Drug-Induced Deficiency (e.g., Methotrexate):** 1 mg to 5 mg daily (avoid administration on the same day as methotrexate).
## Pediatric Dosing
* **Maintenance/Prophylaxis:** 0.1 mg to 0.4 mg daily based on age.
* **Treatment of Deficiency:** 0.5 mg to 1 mg daily.
* **Chronic Hemolytic Anemia:** 0.1 mg to 1 mg daily depending on age (usually 1 mg if >1 year).
## Dose Adjustments
* **Renal/Hepatic Impairment:** No specific dose adjustment necessary; however, monitor hemoglobin/hematocrit and folate levels closely as renal failure patients may have elevated requirements due to dialysis.
## Contraindications
* Hypersensitivity to any component of the formulation.
* **Crucial:** Do not administer as monotherapy in undiagnosed megaloblastic anemia (e.g., pernicious anemia). Folic acid may mask neurological symptoms of Vitamin B12 deficiency (subacute combined degeneration) while allowing progression of neurological damage.
## Adverse Effects
Generally well-tolerated. Rare reports include:
* Gastrointestinal: Nausea, bloating, flatulence.
* Hypersensitivity: Rash, pruritus, or anaphylaxis (rare).
* CNS: Irritability or sleep disturbances (at very high doses).
## Key Drug Interactions
* **Methotrexate:** Folic acid may decrease efficacy. Schedule doses to avoid interference.
* **Anticonvulsants (Phenytoin, Carbamazepine, Valproate):** Folic acid may decrease serum concentrations of these medications, potentially reducing seizure control.
* **Sulfasalazine:** Inhibits intestinal absorption of folate.
## Monitoring
* **Baseline:** Complete blood count (CBC), serum folate, and Vitamin B12 levels.
* **Follow-up:** Reticulocyte count (within 1–2 weeks), Hgb/Hct, and symptom resolution.
* **Safety:** Monitor for neurological symptoms if B12 deficiency is considered possible.
## Clinical Pearls
* In the presence of Vitamin B12 deficiency, folic acid will correct the anemia (hematologic response) but will *not* correct or prevent neurological damage. Ensure B12 status is established before long-term high-dose folic acid therapy.
* Synthetic folic acid has higher bioavailability than dietary folate.
* Dosing for high-risk pregnancies may vary by institutional or jurisdictional clinical guidelines (e.g., CDC vs. ACOG); always defer to local protocols.
