# Fluids in Pediatrics
## Overview
Maintenance fluid therapy is designed to provide water, electrolytes, and glucose to maintain homeostasis in patients unable to meet needs enterally. Over time, the "Holliday-Segar" formula has been challenged due to the risk of iatrogenic hyponatremia associated with hypotonic solutions; current consensus favors isotonic crystalloids.
## Primary Indications
* Maintenance therapy for patients NPO.
* Resuscitation (hypovolemia, shock).
* Replacement of ongoing losses (gastrointestinal, burns).
* Correcting pre-existing deficits.
## Adult Dosing
* **Maintenance:** Generally 1,500–2,500 mL/day depending on size and metabolic rate. Isotonic fluids (0.9% NaCl or Plasmalyte/LR) are preferred over hypotonic solutions (e.g., 0.45% NaCl).
## Pediatric Dosing
* **Maintenance (Holliday-Segar):**
* 1–10 kg: 100 mL/kg/day
* 11–20 kg: 1,000 mL + 50 mL/kg for each kg between 11-20
* >20 kg: 1,500 mL + 20 mL/kg for each kg >20
* *Calculate hourly rate by dividing total daily requirement by 24.*
* **Fluid Selection:** Recent guidelines (AAP/consensus) recommend **isotonic solutions** (0.9% NaCl, Lactated Ringer’s, or Plasma-Lyte) for maintenance to prevent hyponatremia.
* **Resuscitation:** 20 mL/kg bolus of isotonic crystalloid, repeated as clinical status requires.

## Dose Adjustments

## Contraindications
* Fluid overload (edema, congestive heart failure).
* Specific electrolyte imbalances (e.g., avoid high-potassium fluids in severe hyperkalemia or acute renal failure).
## Adverse Effects
* Hyponatremia (associated with hypotonic maintenance fluids).
* Hyperchloremic metabolic acidosis (high-volume 0.9% NaCl).
* Fluid overload/pulmonary edema.
* Hyperglycemia (if using dextrose-heavy solutions without insulin titration).
## Key Drug Interactions
* **Incompatibility:** Many medications are physically incompatible with specific fluid components (e.g., Calcium-containing fluids with Ceftriaxone in neonates; precipitant risk). Always verify Y-site compatibility.
## Monitoring
* **Electrolytes:** Sodium, Potassium, Chloride, Glucose.
* **Output:** Urine output (target 1–2 mL/kg/hour in infants).
* **Physical:** Daily weights, edema, lung sounds, heart rate, and blood pressure.
* **Hydration Status:** Mucous membranes, capillary refill, fontanelle tension.
## Clinical Pearls
* **Isotonic Shift:** Avoid hypotonic maintenance fluids (e.g., D5 1/4NS, D5 1/2NS) due to high risk of hospital-acquired hyponatremia. Isotonic fluids are safer for the vast majority of pediatric patients.
* **Dextrose:** Always consider the need for glucose in maintenance fluids, particularly in neonates and infants <10 kg, to prevent hypoglycemia.
* **Individualization:** Maintenance formulas are estimates; therapy must be refined based on clinical response, ongoing losses, and serial laboratory values.
