# Fluids in Pediatrics
## Overview
Pediatric fluid management requires careful calculation based on maintenance needs, ongoing losses, and resuscitation requirements. Isotonic crystalloids (e.g., 0.9% NaCl or balanced salt solutions like Lactated Ringer’s or Plasma-Lyte) are the standard for resuscitation. Maintenance fluids are typically isotonic to prevent hospital-acquired hyponatremia.
## Primary Indications
* Fluid resuscitation (hypovolemic/septic shock).
* Maintenance therapy (when enteral intake is insufficient).
* Correction of electrolyte imbalances and dehydration.
## Adult Dosing
* **Resuscitation:** 30 mL/kg of isotonic crystalloid rapidly, titrated to clinical response.
* **Maintenance:** Generally 25–35 mL/kg/day (varies by comorbidities).
## Pediatric Dosing
**Resuscitation:** 20 mL/kg bolus of isotonic crystalloid (e.g., 0.9% NaCl or LR) IV/IO, repeated as needed based on clinical status (e.g., heart rate, perfusion, blood pressure). May be reduced to 5–10 mL/kg in cases of suspected cardiac dysfunction.
**Maintenance (Holliday-Segar Method):**
* 1–10 kg: 100 mL/kg/day.
* 11–20 kg: 1000 mL + 50 mL/kg for every kg over 10 kg.
* >20 kg: 1500 mL + 20 mL/kg for every kg over 20 kg.
* *Hourly rate:* Divide the total daily volume by 24.

## Dose Adjustments

## Contraindications
* Fluid overload (e.g., congestive heart failure, some nephritic conditions).
* Severe electrolyte disturbances where specific composition is required (e.g., avoiding potassium in hyperkalemia).
## Adverse Effects
* Fluid overload (pulmonary edema, hypertension).
* Iatrogenic hyponatremia (associated with hypotonic fluids).
* Hyperchloremic metabolic acidosis (high-volume 0.9% NaCl).
* Extravasation injury.
## Key Drug Interactions
* **Calcium/Ceftriaxone:** Do not co-administer Ringer’s-type solutions (containing Ca++) with Ceftriaxone in neonates (<28 days) due to risk of precipitation.
* **Incompatible additives:** Consult compatibility charts for electrolyte additives (e.g., potassium, magnesium) to prevent precipitation.
## Monitoring
* Strict intake and output (I&O).
* Body weight (daily).
* Serum electrolytes, glucose, and acid-base status.
* Clinical assessment: signs of dehydration (skin turgor, mucous membranes, fontanelle) or fluid overload (crackles, edema, hepatomegaly).
## Clinical Pearls
* "Maintenance" is an estimate; clinical monitoring is superior to any mathematical formula.
* Avoid standard maintenance fluids for shock; use weight-based crystalloid boluses.
* Monitor serum glucose carefully in neonates and small infants, as typical maintenance fluids may not provide adequate glucose, leading to hypoglycemia.
* Balanced crystalloids (LR, Plasma-Lyte) are increasingly preferred over 0.9% saline to avoid hyperchloremic acidosis.
