# Fluids in Pediatrics
## Overview
Fluid therapy in pediatrics involves isotonic crystalloids (e.g., 0.9% Normal Saline, Lactated Ringer’s, or Plasma-Lyte) for resuscitation and maintenance. Balanced crystalloids (Lactated Ringer’s or Plasma-Lyte) are increasingly preferred over 0.9% Normal Saline to reduce the risk of hyperchloremic metabolic acidosis.
## Primary Indications
* **Resuscitation:** Hypovolemic shock, sepsis, severe dehydration, and trauma.
* **Maintenance:** Hydration for patients unable to tolerate enteral intake.
* **Correction:** Electrolyte disturbances and fluid loss replacement.

## Pediatric Dosing
### Resuscitation
* **Standard dose:** 20 mL/kg of an isotonic crystalloid administered as a rapid bolus.
* **Caveats:** In septic shock or DKA, current guidelines (e.g., Surviving Sepsis, ISPAD) often recommend smaller, more frequent boluses (e.g., 10 mL/kg) and frequent reassessment to avoid fluid overload.
### Maintenance Fluids (Holliday-Segar Method)
* **0–10 kg:** 100 mL/kg/day.
* **11–20 kg:** 1,000 mL + 50 mL/kg for every kg over 10 kg.
* **>20 kg:** 1,500 mL + 20 mL/kg for every kg over 20 kg (Max 2,400 mL/day).

* **Laboratory:** Serum electrolytes (Na, K, Cl), pH, CO2, and output (Target 1–2 mL/kg/hr for infants; 0.5–1 mL/kg/hr for older children).
## Clinical Pearls
* **Isotonic Preference:** Avoid using hypotonic maintenance fluids (e.g., 0.2% NaCl) in hospitalized children as they carry a high risk of iatrogenic hyponatremia, especially in the presence of ADH secretion (stress, pain, nausea).
* **Local Protocols:** Fluids must always be prescribed according to the *specific institutional clinical practice guidelines*, particularly regarding the use of additives (potassium, chloride, glucose).
* **Sepsis:** Fluid boluses should be titrated to clinical response, not just volume, to prevent fluid-associated myocardial dysfunction.
