# Fluids in Pediatrics
## Overview
Pediatric intravenous fluid therapy requires a precise balance of maintenance needs (to support physiologic function) and replacement needs (to address deficits or ongoing losses). Isotonic crystalloids (e.g., 0.9% Normal Saline or Balanced Salt Solutions like Lactated Ringer's) are currently preferred over hypotonic solutions to reduce the risk of iatrogenic hyponatremia.
## Primary Indications
- Maintenance of hydration in patients unable to meet needs enterally.
- Resuscitation of shock/hypovolemia.
- Replacement of ongoing losses (e.g., GI losses, third-spacing).
- Correction of electrolyte abnormalities.
## Adult Dosing

## Pediatric Dosing

## Adverse Effects

## Key Drug Interactions

## Monitoring
- **Clinical:** Daily weights (most sensitive marker), intake and output (I&O), heart rate, blood pressure, capillary refill time, and neurological status.
- **Laboratory:** Serum electrolytes, BUN, creatinine, and glucose at intervals determined by clinical status and stability.

- **Balanced Solutions:** Current consensus favors balanced isotonic solutions (LR or Plasma-Lyte) over 0.9% NaCl for resuscitation to minimize the risk of acid-base disturbances.
- **Glucose:** Add dextrose (e.g., D5) to maintenance fluids in neonates and young infants to prevent hypoglycemia.
- **Neurological Focus:** Always prioritize isotonic fluids in children with potential increased intracranial pressure.
