# Fluids in Pediatrics
## Overview
Pediatric intravenous fluid therapy aims to maintain homeostasis (maintenance) and correct deficits (resuscitation or replacement). Isotonic balanced crystalloids (e.g., Plasma-Lyte, Lactated Ringer’s) are increasingly preferred over 0.9% NaCl to mitigate the risk of hyperchloremic metabolic acidosis.
## Primary Indications
* **Maintenance:** Baseline requirements for water and electrolytes in patients unable to meet needs enterally.
* **Resuscitation:** Correction of hypovolemia, shock, or severe dehydration.
* **Replacement:** Correction of ongoing abnormal losses (e.g., NG suction, fistula output).
## Adult Dosing

## Pediatric Dosing
**Maintenance (Holliday-Segar Method):**
* **0–10 kg:** 100 mL/kg/day (or 4 mL/kg/hr)
* **11–20 kg:** 1,000 mL + 50 mL/kg for each kg >10 (or 2 mL/kg/hr)
* **>20 kg:** 1,500 mL + 20 mL/kg for each kg >20 (or 1 mL/kg/hr)
* *Maximum maintenance:* Generally 2,400 mL/day.
**Resuscitation:**
* **Isotonic Crystalloid Bolus:** 20 mL/kg IV bolus over 5–20 minutes. May repeat as clinically indicated. In septic shock, follow age-specific guidelines (e.g., early goal-directed therapy).
## Dose Adjustments
* **Renal/Cardiac Impairment:** Reduce maintenance rates or volumes significantly (e.g., 50–75% of calculated needs) to avoid fluid overload, pulmonary edema, or heart failure.
* **Syndrome of Inappropriate Antidiuretic Hormone (SIADH):** Fluid restrict to 50–75% of maintenance requirements.
## Contraindications
* **Severe Fluid Overload:** Pulmonary edema or congestive heart failure.
* **Specific Compositions:** Avoid Lactated Ringer’s in patients with severe hyperkalemia or suspected lactic acidosis (though rarely contraindicated in modern practice).
## Adverse Effects
* **Iatrogenic Hyponatremia:** Associated with the use of hypotonic maintenance fluids (e.g., D5W/0.2% NaCl).

## Monitoring
* **Electrolytes:** Frequent assessment of serum Na, K, Cl, and Glucose.
* **Weight:** Daily weights to assess fluid status.
* **Fluid Balance:** Strict inputs and outputs (I&Os).
* **Clinical Signs:** Assessment for mucosal dryness, capillary refill, urine output (>1 mL/kg/hr), and signs of fluid overload (crackles, hepatomegaly, edema).
## Clinical Pearls
* **Hypertonic Solutions:** 3% NaCl is used for symptomatic hyponatremia/seizure, typically 2–4 mL/kg per dose.
* **Isotonic Preference:** Avoid hypotonic maintenance fluids (e.g., 0.2% NaCl) in hospitalized children due to documented risk of hyponatremia.
* **DKA Protocol:** Specific fluid calculations are required for DKA to avoid cerebral edema; consult local institutional DKA pathways.
