# Fluids in Pediatrics
## Overview
Pediatric fluid therapy is categorized into maintenance, replacement (deficit/bolus), and ongoing loss management. Isotonic crystalloids (0.9% NaCl or balanced salt solutions like Lactated Ringer’s/Plasma-Lyte) are the standard of care for resuscitation. Hypotonic solutions are generally avoided to prevent hyponatremia.
## Primary Indications
* **Resuscitation:** Hypovolemic shock, sepsis, severe dehydration.
* **Maintenance:** When oral intake is inadequate.
* **Replacement:** Correcting deficits due to losses (GI, renal, insensible).
## Adult Dosing

## Pediatric Dosing
**Resuscitation (Bolus):**

* In cardiogenic shock or specific neonate populations, use 5–10 mL/kg cautiously.
**Maintenance (Holliday-Segar Formula):**

* 11–20 kg: 1000 mL + 50 mL/kg for each kg over 10 kg.
* >20 kg: 1500 mL + 20 mL/kg for each kg over 20 kg.

## Clinical Pearls
* **"Isotonic is safer":** Standard use of 1/4 or 1/2 normal saline for maintenance is increasingly discouraged due to hospital-acquired hyponatremia. Ensure glucose is provided in maintenance fluids for neonates and young infants to prevent hypoglycemia.
* **Balanced Solutions:** LR or Plasma-Lyte are often preferred over 0.9% NaCl for large-volume resuscitation to avoid hyperchloremic acidosis.
