# Fluids in Pediatrics
## Overview
Pediatric fluid management focuses on maintaining water and electrolyte homeostasis while preventing complications such as hyponatremia and fluid overload. The shift in clinical practice has moved toward using isotonic, balanced electrolyte solutions (e.g., Plasma-Lyte or Lactated Ringer’s) over hypotonic solutions (e.g., 0.45% NaCl) for maintenance to reduce the risk of hospital-acquired hyponatremia.
## Primary Indications
* Maintenance hydration for patients unable to tolerate enteral intake.
* Replacement of ongoing losses (gastrointestinal, renal, or third-space).
* Resuscitation in hypovolemia or shock.

## Pediatric Dosing
Maintenance fluid requirements are typically calculated via the Holliday-Segar method:
* **1–10 kg:** 100 mL/kg/day.
* **11–20 kg:** 1,000 mL + 50 mL/kg for each kg over 10.
* **>20 kg:** 1,500 mL + 20 mL/kg for each kg over 20 (max 2,400–2,500 mL/day).
**Resuscitation:**

## Key Drug Interactions
* **Drug Compatibility:** Many medications are incompatible with fluids containing calcium (e.g., LR) or high-glucose concentrations. Always consult a Y-site compatibility chart (e.g., Trissel's) before co-administration via the same line.
## Monitoring
* **Clinical:** Daily weights (most sensitive marker for fluid balance), intake and output (I/O) balance, heart rate, perfusion, and urine output (target >1 mL/kg/hr).
* **Laboratory:** Serum electrolytes (Na, K, Cl), BUN/Creatinine, and glucose (especially in infants and neonates).
## Clinical Pearls
* **Isotonic Preference:** Most modern pediatric guidelines recommend isotonic fluids for routine maintenance to avoid the risk of ADH-mediated hyponatremia.
* **Glucose Needs:** Infants and neonates have limited glycogen stores and may require dextrose-containing fluids (e.g., D5-0.9% NaCl) to prevent hypoglycemia.
* **Weight-Based Accuracy:** Always re-weigh the patient daily; fluid requirements change dynamically with clinical status.
