# Ferrous ascorbate
## Overview
Ferrous ascorbate is an oral **ferrous (Fe²⁺) iron salt combined with ascorbate (vitamin C)**. It is used to replace iron in iron-deficiency states.
- Dose must be calculated as **elemental iron**, not the total ferrous ascorbate salt.
- Elemental iron content varies by product; verify the package or local formulary. Do **not** assume that “100 mg ferrous ascorbate” equals 100 mg elemental iron.
- Vitamin C may improve absorption, but additional vitamin C is usually unnecessary.
- Use only after iron deficiency is reasonably established or strongly suspected; iron can worsen some non–iron-deficiency anemias and iron-overload disorders.
## Primary Indications
- Treatment of iron-deficiency anemia.
- Treatment of iron deficiency without anemia when clinically indicated.
- Prevention of iron deficiency in selected high-risk patients, including pregnancy or inadequate dietary intake.
- Increased iron requirements, such as during pregnancy, lactation, or recovery from blood loss.
## Adult Dosing
Doses below refer to **elemental iron**:
- **Iron-deficiency anemia:**
**100–200 mg elemental iron/day** orally, usually once daily or in divided doses. Many patients tolerate and absorb **40–65 mg once daily or on alternate days** better than multiple daily doses.
- **Iron deficiency without anemia or prophylaxis:**
Commonly **30–60 mg elemental iron/day**; local protocols may differ.
- Continue treatment for approximately **3 months after hemoglobin normalizes** to replenish iron stores, guided by ferritin and clinical response.
- There is no universally established ferrous-ascorbate-specific maximum. In routine oral therapy, **200 mg elemental iron/day** is a commonly used upper treatment dose; higher doses should be specialist- or protocol-directed because adverse effects increase and absorption does not necessarily improve.
Administration:
- Take on an empty stomach if tolerated.
- If gastrointestinal effects occur, take with a small amount of food; absorption may be reduced.
- Avoid taking with milk, calcium, tea, coffee, antacids, or high-fiber foods.
- Swallow tablets whole unless the product is specifically intended to be chewed or dispersed.
## Pediatric Dosing
Doses refer to **elemental iron**:
- **Treatment of iron-deficiency anemia:**
**3–6 mg/kg/day orally**, once daily or divided; use the lower end for mild disease and the higher end for severe anemia or selected high-risk situations.
- **Prevention in term infants/children at risk:**
Commonly **1–2 mg/kg/day**, according to age, diet, and local guidance.
- **Preterm or low-birth-weight infants:**
Often **2–4 mg/kg/day**, beginning when clinically appropriate; neonatal protocols should be followed.
- A commonly used maximum for pediatric treatment is **200 mg elemental iron/day**, but product labeling and local pediatric protocols take precedence.
- Do not exceed the prescribed dose. Accidental ingestion can cause life-threatening toxicity, particularly in young children.
## Dose Adjustments
- **Renal impairment:** No routine dose adjustment is established for oral ferrous ascorbate. Treat the cause of anemia and consider functional iron deficiency, inflammation, or erythropoiesis-stimulating-agent protocols.
- **Hepatic impairment:** No standard adjustment; avoid unnecessary iron in chronic liver disease or suspected iron overload.
- **Malabsorption, ongoing blood loss, severe anemia, intolerance, or inadequate response:** Reassess diagnosis, adherence, bleeding, and absorption. Intravenous iron may be more appropriate under specialist guidance.
- **Pregnancy:** Common prophylactic doses are approximately **30–60 mg elemental iron/day**; treatment dosing is generally **100–200 mg/day**, guided by obstetric and local protocols.
- **Older adults:** No age-based adjustment, but investigate occult blood loss and alternative causes of anemia.
## Contraindications
- Hypersensitivity to ferrous ascorbate or excipients.
- Iron-overload disorders, including hemochromatosis or hemosiderosis, unless specifically directed by a specialist.
- Anemias not caused by iron deficiency, including some hemolytic, sideroblastic, or thalassemia-related anemias.
- Repeated transfusion-associated iron overload.
- Use caution with active gastrointestinal ulceration, severe inflammatory bowel disease, or significant malabsorption.
## Adverse Effects
Common, dose-related effects:
- Nausea, abdominal discomfort, dyspepsia.
- Constipation or diarrhea.
- Vomiting.
- Dark or black stools.
- Metallic taste.
- Heartburn.
Other considerations:
- Liquid preparations may stain teeth; administer toward the back of the mouth and rinse afterward.
- Severe abdominal pain, persistent vomiting, lethargy, metabolic acidosis, or bloody diarrhea after ingestion may indicate acute iron poisoning and requires emergency treatment.
- Iron can exacerbate gastrointestinal symptoms and may obscure assessment of gastrointestinal bleeding.
## Key Drug Interactions
Separate oral iron from interacting medicines when clinically feasible:
- **Levothyroxine:** Separate by at least **4 hours**; monitor thyroid function.
- **Tetracyclines:** Separate by at least **2–4 hours**.
- **Fluoroquinolones:** Separate by at least **2 hours before or 4–6 hours after** iron, depending on the antibiotic and local labeling.
- **Bisphosphonates:** Separate by at least **2 hours**; follow the bisphosphonate-specific instructions.
- **Antacids, calcium, magnesium, zinc, and other mineral supplements:** Separate by at least **2 hours**.
- **Proton-pump inhibitors and chronic acid suppression:** May reduce iron absorption.
- **Methyldopa:** Iron may reduce absorption and antihypertensive effect; separate administration and monitor blood pressure.
- **Chloramphenicol:** May reduce hematologic response to iron.
- **Penicillamine:** Separate by at least **2 hours**.
- Food, especially calcium-containing foods, tea, coffee, and phytate-rich foods, can reduce absorption.
## Monitoring
- Baseline and follow-up: CBC with hemoglobin, hematocrit, MCV, reticulocyte count when appropriate, ferritin, and transferrin saturation.
- Recheck hemoglobin after approximately **2–4 weeks** in treated anemia. A rise of roughly **1–2 g/dL over 2–4 weeks** is generally expected if adherence, absorption, and diagnosis are appropriate.
- Continue monitoring until hemoglobin normalizes and iron stores are replenished.
- Investigate lack of response for nonadherence, incorrect dosing of elemental iron, ongoing blood loss, malabsorption, inflammation, or an alternative diagnosis.
- Avoid routine iron therapy without monitoring in patients at risk for iron overload.
- Monitor children closely for accidental ingestion.
## Clinical Pearls
- Prescribe and document the dose as **mg of elemental iron**.
- Ferrous ascorbate products are not interchangeable by tablet strength; confirm each product’s elemental iron equivalence.
- Once-daily or alternate-day dosing may improve tolerability and may enhance absorption compared with frequent high-dose administration, although local protocols vary.
- A response to iron does not replace evaluation for the cause of deficiency. In adults, assess for menstrual, gastrointestinal, dietary, surgical, or other blood loss as clinically appropriate.
- Iron is absorbed best on an empty stomach, but taking it with a small amount of food is reasonable if gastrointestinal intolerance limits adherence.
- Keep all iron products locked away from children. Any suspected overdose requires immediate poison-center or emergency evaluation; do not wait for symptoms.
