# Ferrous Ascorbate
## Overview
Ferrous ascorbate is a chelated complex of iron and ascorbic acid (Vitamin C). The addition of ascorbic acid creates an acidic microenvironment in the gut, which enhances the reduction of ferric iron to the more absorbable ferrous state, theoretically improving bioavailability and reducing gastrointestinal irritation compared to standard ferrous sulfate.
## Primary Indications
* Treatment of iron deficiency anemia.
* Prophylaxis of iron deficiency, particularly during pregnancy and lactation.
## Adult Dosing
* **Iron Deficiency Anemia:** 100–200 mg of elemental iron daily, divided into 1–2 doses.
* **Maintenance/Prophylaxis:** 50–100 mg of elemental iron daily.
* *Note:* Exact formulation content (mg of elemental iron per tablet/liquid) varies widely by manufacturer; confirm elemental iron content before prescribing.
## Pediatric Dosing
* **Treatment:** 3–6 mg of elemental iron/kg/day, divided into 1–3 doses.

## Dose Adjustments
* **Renal Impairment:** Generally not required, but use with caution in patients with chronic kidney disease (CKD) who may require parenteral iron if oral absorption is impaired or if on dialysis.
* **Hepatic Impairment:** No specific adjustments, but monitor for iron overload in patients with pre-existing hepatic iron accumulation.
## Contraindications
* Hypersensitivity to iron or ascorbic acid.
* Conditions with iron overload (e.g., hemochromatosis, hemosiderosis).
* Hemolytic anemia (unless iron deficiency is proven).
* Repeated blood transfusions.
## Adverse Effects
* **Gastrointestinal:** Constipation, dark/black stools, nausea, abdominal pain, diarrhea.
* **Systemic:** Teeth staining (in liquid formulations; advise rinsing mouth after use).
* **Overdose:** Potentially fatal in children; symptoms include severe abdominal pain, vomiting (often hematemesis), diarrhea (often bloody), hypotension, and shock.
## Key Drug Interactions

* **Fluoroquinolones/Tetracyclines:** Iron forms insoluble chelates; separate doses by at least 2–4 hours.
* **Antacids/PPIs/H2 Blockers:** Elevate gastric pH and significantly reduce iron absorption; separate doses by 2 hours.
* **Thyroid hormones (Levothyroxine):** Iron reduces absorption; separate doses by at least 4 hours.
## Monitoring
* **Baseline:** Complete Blood Count (CBC), serum ferritin, transferrin saturation.
* **Efficacy:** Reticulocyte count (7–10 days post-initiation), hemoglobin/hematocrit (4 weeks post-initiation).
* **Safety:** Monitor for GI distress and adherence. Assess for signs of chronic iron overload if therapy is extended.
## Clinical Pearls
* **Absorption:** Best taken on an empty stomach (1 hour before or 2 hours after meals) to maximize absorption; however, if GI side effects are intolerable, taking with food is acceptable to improve adherence despite reduced absorption.
* **Ascorbate Advantage:** While marketed for superior absorption, confirm that the cost-benefit justifies its use over less expensive ferrous sulfate/fumarate options for the specific patient.
* **Stool color:** Educate patients that dark or black stools are a harmless side effect of iron therapy.
