# Ferrous Ascorbate
## Overview
Ferrous ascorbate is an iron salt complexed with ascorbic acid (vitamin C). The addition of ascorbic acid lowers intragastric pH and acts as a reducing agent, converting ferric iron to the more absorbable ferrous state and enhancing overall bioavailability compared to conventional iron salts.
## Primary Indications
* Treatment of iron deficiency anemia (IDA).
* Prophylaxis of iron deficiency in high-risk populations (e.g., pregnancy, chronic blood loss).
## Adult Dosing
* **Treatment of IDA:** 100 mg elemental iron to 200 mg elemental iron daily, typically administered in 1 or 2 divided doses.
* **Prophylaxis:** 50 mg to 100 mg elemental iron daily.
## Pediatric Dosing
* **Treatment of IDA:** 3 mg to 6 mg of elemental iron/kg/day, administered in 1 to 3 divided doses.
* *Note:* Do not exceed adult maximums. Dosing varies significantly by local institutional protocol and severity of anemia; consult standardized weight-based pediatric formularies.
## Dose Adjustments
* **Renal Impairment:** No specific adjustment required; however, use with caution in patients with chronic kidney disease (CKD) who may require intravenous iron therapy if oral absorption is impaired by systemic inflammation.
* **Hepatic Impairment:** No specific adjustment required; use with caution in patients with iron overload syndromes (e.g., hemochromatosis).
## Contraindications
* Hypersensitivity to iron or ascorbic acid.
* Hemochromatosis, hemosiderosis, or hemolytic anemias.
* Patients receiving repeated blood transfusions.
* Active peptic ulcer disease or inflammatory bowel disease (may exacerbate symptoms).
## Adverse Effects
* **Gastrointestinal:** Nausea, abdominal pain, constipation, diarrhea, and dark/black stools (expected and harmless).
* **General:** Staining of teeth (if liquid form is used; suggest using a straw and rinsing mouth).
* *Note:* GI adverse effects are often dose-dependent and may improve with lower, alternate-day dosing.
## Key Drug Interactions
* **Antacids, H2 blockers, PPIs:** Decreased absorption; separate administration by at least 2 hours.
* **Fluoroquinolones and Tetracyclines:** Iron chelates with these antibiotics, significantly reducing their absorption; separate doses by 2 to 4 hours.
* **Levodopa/Methyldopa:** Iron may reduce absorption; monitor efficacy.
* **Thyroid Hormones (Levothyroxine):** Separate administration by at least 4 hours to avoid reduced thyroid hormone efficacy.
## Monitoring
* **Baseline:** Complete Blood Count (CBC), serum ferritin, and percent transferrin saturation (TSAT).
* **Response:** Reticulocyte count (7–10 days post-initiation), hemoglobin/hematocrit (4 weeks post-initiation).
* **Safety:** Monitor for worsening GI symptoms or signs of iron overload in chronic users.
## Clinical Pearls
* **Bioavailability:** Ascorbic acid increases iron absorption; however, the complex is still best absorbed on an empty stomach. If GI intolerance occurs, it may be taken with food, accepting a slight reduction in absorption.
* **Adherence:** Iron therapy often requires 3–6 months to replenish systemic iron stores after hemoglobin levels normalize.
* **Toxicity Risk:** Iron supplements are a leading cause of fatal pediatric poisoning; keep tablets securely stored away from children.
