# Ferrous Ascorbate
## Overview
Ferrous ascorbate is a chelated form of iron formed by the reaction of ferrous iron and ascorbic acid (Vitamin C). The presence of ascorbic acid facilitates the reduction of ferric iron to the more absorbable ferrous state and creates a complex that enhances iron solubility and bioavailability in the gastrointestinal tract.
## Primary Indications
* Treatment and prophylaxis of iron-deficiency anemia (IDA).
* Treatment of iron deficiency during pregnancy and lactation.
## Adult Dosing
Standard dosing is based on elemental iron content, which varies by manufacturer formulation.
* **Typical Dose:** 1 tablet (usually 100 mg elemental iron) orally once or twice daily.
* **Maximum Dose:** Generally 200 mg of elemental iron daily, though higher doses increase GI intolerance without proportional gains in absorption due to hepcidin-mediated blockade.
## Pediatric Dosing
* **Treatment of IDA:** 3–6 mg/kg/day of elemental iron in 1–3 divided doses.
* *Note:* Optimal dosing is highly dependent on local clinical protocols and patient-specific weight/hemoglobin levels. Always verify weight-based calculations against institutional guidelines.
## Dose Adjustments

## Clinical Pearls
* **Increased Absorption:** Ascorbic acid is built into the complex to optimize acidity; however, supplemental Vitamin C is not typically required.
* **Administration:** It is ideally taken on an empty stomach to maximize absorption, but taking with food may be necessary to improve GI tolerability.
* **Toxicity:** Pediatric iron poisoning is a medical emergency. Keep containers tightly closed and out of reach of children.
* **Compliance:** If daily dosing causes significant GI distress, once-daily or alternate-day dosing is often as effective for raising hemoglobin while significantly improving patient adherence.
