# Ezivac enema
## Overview
Ezivac enema is a hyperosmotic laxative containing Sodium Phosphate. It works by drawing water into the colon, which softens the stool and stimulates bowel evacuation.
## Primary Indications
* Relief of occasional constipation.
* Bowel preparation prior to diagnostic procedures or surgery.
## Adult Dosing
* **Constipation:** Typically, one 4.5 ounce (133 mL) enema administered rectally as a single dose.
* **Bowel Preparation:** Consult specific institutional protocol. Dosing may involve multiple enemas or combination with oral agents.
## Pediatric Dosing
Use in children under 12 years of age is generally not recommended due to the risk of electrolyte disturbances and dehydration. Safety and efficacy have not been established. Consult a pediatrician for appropriate alternatives.
## Dose Adjustments
No specific dose adjustments are required in hepatic or renal impairment, but caution is advised due to potential for electrolyte imbalances.
## Contraindications
* Congenital or acquired megacolon.
* Intestinal obstruction or perforation.
* Severe renal impairment.
* Congestive heart failure (CHF).
* Children under 12 years of age.
* Patients with ileostomy or colostomy.
* Anuria.
* Hypersensitivity to sodium phosphate.
## Adverse Effects
* **Common:** Abdominal cramping, nausea, vomiting, rectal irritation, bloating.
* **Serious:** Electrolyte imbalances (hyperphosphatemia, hypocalcemia, hypernatremia, dehydration), seizures, cardiac arrhythmias, renal injury.
## Key Drug Interactions
* **Diuretics and Antihypertensives:** Increased risk of electrolyte imbalance and dehydration.
* **Drugs affecting renal function:** May exacerbate renal toxicity.
## Monitoring
* Monitor for signs and symptoms of dehydration and electrolyte imbalance (e.g., thirst, dry mouth, dizziness, muscle cramps, weakness, confusion).
* Assess renal function in patients with risk factors.
## Clinical Pearls
* Administer as directed; do not retain the enema for longer than necessary.
* Encourage fluid intake after administration.
* Avoid overuse, as it can lead to dependence.
* This product should not be used for more than one week unless directed by a healthcare professional.
