# Ecospirin (Aspirin, Enteric-Coated)
## Overview
Ecospirin is an enteric-coated formulation of aspirin, a nonsteroidal anti-inflammatory drug (NSAID) with analgesic, anti-inflammatory, and antipyretic properties. The enteric coating is designed to delay the release of aspirin until it reaches the small intestine, potentially reducing gastric irritation.
## Primary Indications
* **Cardiovascular Prevention:** Primary and secondary prevention of myocardial infarction and stroke in patients with established cardiovascular disease or risk factors.
* **Rheumatic Fever Prophylaxis:** Adjunct in the management of acute rheumatic fever.
* **Pain and Inflammation:** Relief of mild to moderate pain and reduction of fever (less common indication for enteric-coated formulations due to slower onset).
## Adult Dosing
* **Cardiovascular Prevention:**
* **Primary Prevention:** 75-100 mg once daily. Higher doses may be considered in select high-risk populations based on clinical judgment and risk assessment.
* **Secondary Prevention:** 75-325 mg once daily. Often initiated at a higher dose (e.g., 325 mg) for acute events, then transitioned to chronic low-dose therapy.
* **Rheumatic Fever Prophylaxis:** 50-100 mg/kg/day divided every 4-6 hours, not to exceed 4 grams/day.
* **Pain and Inflammation:** Dosing varies but typically starts at 325-650 mg every 4-6 hours as needed. The enteric coating may delay onset of pain relief.
## Pediatric Dosing
* **Rheumatic Fever Prophylaxis:** 50-100 mg/kg/day divided every 4-6 hours. Maximum dose should not exceed adult maximums for similar indications.
* **Kawasaki Disease:** High dose (80-100 mg/kg/day divided every 4-6 hours) during the acute phase, followed by low dose (3-5 mg/kg/day once daily) for at least 6-8 weeks. **Note:** Specific dosing protocols for Kawasaki disease may vary; consult local pediatric guidelines.
* **Fever and Pain:** Aspirin is generally **avoided** in children and adolescents with viral illnesses due to the risk of Reye's Syndrome. When absolutely necessary and no other alternatives exist, pediatric dosing is typically 10-15 mg/kg/dose every 4-6 hours.
## Dose Adjustments
* **Renal Impairment:** Use with caution. Dose reduction may be considered in severe renal impairment. Generally avoided in end-stage renal disease.
* **Hepatic Impairment:** Use with caution. Dose reduction may be necessary.
* **Elderly:** Increased risk of gastrointestinal bleeding and other adverse effects. Lower doses and increased monitoring are recommended.
## Contraindications
* Hypersensitivity to aspirin, salicylates, or NSAIDs.
* History of asthma, urticaria, or other allergic-type reactions precipitated by aspirin or other NSAIDs.
* Active peptic ulceration or gastrointestinal bleeding.
* Children and teenagers with viral infections (due to risk of Reye's Syndrome).
* Severe hepatic or renal impairment.
* Concomitant use with methotrexate at high doses.
## Adverse Effects
* **Gastrointestinal:** Dyspepsia, heartburn, nausea, vomiting, abdominal pain, erosions, ulcers, bleeding, perforation.
* **Hematologic:** Increased bleeding time, bruising, petechiae, epistaxis, prolonged hemorrhage.
* **Hypersensitivity:** Bronchospasm, angioedema, urticaria, rash.
* **Ototoxicity:** Tinnitus, hearing loss (more common with higher doses).
* **Reye's Syndrome:** In children and adolescents with viral infections.
## Key Drug Interactions
* **Anticoagulants (e.g., warfarin, heparin, DOACs):** Increased risk of bleeding.
* **Other NSAIDs and Corticosteroids:** Increased risk of gastrointestinal bleeding and ulceration.
* **Corticosteroids:** Increased risk of gastrointestinal bleeding.
* **SSRIs and SNRIs:** Increased risk of bleeding, particularly gastrointestinal.
* **Methotrexate:** Increased methotrexate toxicity (especially at high aspirin doses).
* **Antihypertensives (e.g., ACE inhibitors, ARBs, beta-blockers, diuretics):** Aspirin may reduce their efficacy.
* **Uricosuric agents (e.g., probenecid):** Aspirin may decrease their effectiveness.
* **Valproic acid:** Aspirin may displace valproic acid from plasma protein binding.
## Monitoring
* **Gastrointestinal:** Monitor for signs of bleeding (melena, hematemesis, abdominal pain), occult blood in stool.
* **Bleeding:** Monitor for signs of increased bruising or bleeding, especially if on concomitant anticoagulants or antiplatelets.
* **Renal function:** Monitor BUN and creatinine, particularly in patients with risk factors for renal impairment or those on high doses.
* **Hepatic function:** Monitor LFTs, especially with long-term or high-dose use.
* **Tinnitus:** May indicate elevated aspirin levels; consider dose reduction.
## Clinical Pearls
* Enteric coating delays absorption and may reduce gastric irritation but also delays onset of action. Swallow tablets whole; do not crush, chew, or break.
* Use the lowest effective dose, especially for cardiovascular prevention.
* Aspirin should be discontinued at least 5-7 days before elective surgery due to its irreversible antiplatelet effect.
* In patients with established cardiovascular disease, aspirin is often used in combination with other antiplatelet agents (e.g., clopidogrel) for dual antiplatelet therapy (DAPT) following acute coronary syndromes or percutaneous coronary intervention. DAPT duration is protocol-dependent.
* Patient counseling on signs of bleeding and gastrointestinal upset is crucial. Risk of bleeding increases with age and higher doses.
