# Ecospirin (Aspirin, Enteric-Coated)
## Overview
Ecospirin is an enteric-coated formulation of aspirin, a nonsteroidal anti-inflammatory drug (NSAID) with antiplatelet, analgesic, and antipyretic properties. The enteric coating is designed to delay absorption until the drug reaches the small intestine, reducing gastric irritation.
## Primary Indications
* **Cardiovascular Prevention:** Secondary prevention of myocardial infarction, stroke, and transient ischemic attack; primary prevention in select high-risk individuals.
* **Analgesia and Antipyresis:** Mild to moderate pain and fever (less common use due to availability of other NSAIDs with potentially better gastrointestinal safety profiles for these indications).
* **Rheumatic Fever Prophylaxis:** Long-term use for prevention of recurrences.
* **Kawasaki Disease:** Treatment of acute phase and prevention of coronary artery aneurysms.
## Adult Dosing
* **Cardiovascular Prevention:** Typically 75-325 mg once daily. Lower doses (e.g., 81 mg) are common for long-term use. Higher doses (e.g., 325 mg) may be used initially post-event.
* **Analgesia/Antipyresis:** 325-650 mg every 4-6 hours as needed. Maximum daily dose generally 4000 mg.
* **Rheumatic Fever Prophylaxis:** 650 mg every 4-6 hours. Long-term doses vary, often 200-300 mg daily.
* **Kawasaki Disease:** Acute phase: 80-100 mg/kg/day divided every 6 hours. After fever subsides: 3-5 mg/kg once daily.
## Pediatric Dosing
Dosing is typically weight-based and indication-specific. Aspirin should **NOT** be used in children and teenagers with viral infections (e.g., influenza, varicella) due to the risk of Reye's syndrome.
* **Kawasaki Disease:** As stated in Adult Dosing.
* **Juvenile Idiopathic Arthritis (historical use, not preferred):** 60-100 mg/kg/day divided every 4-6 hours. Maximum daily dose typically 4000 mg.
## Dose Adjustments
No specific dose adjustments are typically recommended for renal or hepatic impairment. However, caution and monitoring are advised, especially in severe dysfunction.
## Contraindications
* Hypersensitivity to aspirin, salicylates, NSAIDs, or any component of the formulation.
* Active peptic ulceration or gastrointestinal bleeding.
* History of aspirin-induced asthma, urticaria, or other allergic type reactions.
* Children and teenagers with viral infections (risk of Reye's syndrome).
* Severe hepatic impairment.
* Severe renal impairment (CrCl < 30 mL/min).
* Use in the third trimester of pregnancy.
## Adverse Effects
* **Gastrointestinal:** Dyspepsia, heartburn, nausea, vomiting, abdominal pain, GI bleeding, ulceration.
* **Hematologic:** Increased bleeding time, petechiae, ecchymosis, epistaxis, prolonged hemorrhage from wounds.
* **Dermatologic:** Rash, pruritus, urticaria.
* **Otic:** Tinnitus, hearing loss (especially with higher doses).
* **Respiratory:** Bronchospasm, asthma exacerbation (in susceptible individuals).
* **Other:** Reye's syndrome (in children/teenagers with viral infections).
## Key Drug Interactions
* **Anticoagulants (e.g., warfarin, heparin, DOACs):** Increased risk of bleeding.
* **Other NSAIDs and Salicylates:** Increased risk of GI toxicity and bleeding.
* **Corticosteroids:** Increased risk of GI ulceration and bleeding.
* **Methotrexate:** Aspirin may decrease methotrexate clearance, increasing toxicity.
* **Uricosuric agents (e.g., probenecid):** Aspirin may decrease the efficacy of uricosuric agents.
* **Antihypertensives (e.g., ACE inhibitors, ARBs, beta-blockers):** NSAIDs, including aspirin, can reduce their antihypertensive effect and increase the risk of renal impairment.
* **Selective Serotonin Reuptake Inhibitors (SSRIs):** Increased risk of bleeding.
## Monitoring
* **Bleeding:** Monitor for signs/symptoms of GI bleeding (melena, hematemesis) and other bleeding (bruising, epistaxis).
* **Renal function:** Particularly in patients with pre-existing renal disease or receiving concomitant nephrotoxic agents.
* **Liver function:** In patients with hepatic impairment or those on long-term therapy.
* **Tinnitus/Hearing:** May indicate excessive dosage.
## Clinical Pearls
* The enteric coating minimizes gastric upset but does not eliminate the risk of GI bleeding or ulceration.
* For optimal cardiovascular benefit, aspirin should be taken consistently every day.
* Patients should be advised to avoid concomitant use of other NSAIDs unless specifically directed by their healthcare provider.
* If a dose is missed, instruct patients to take it as soon as remembered, unless it is close to the time for the next scheduled dose. Do not double doses.
* Due to the potential for serious adverse effects like Reye's syndrome, aspirin is generally contraindicated in children and adolescents, especially during viral illnesses.
