# Ecospirin (Aspirin, Enteric-Coated)
## Overview
Ecospirin is an enteric-coated formulation of acetylsalicylic acid (ASA), a nonsteroidal anti-inflammatory drug (NSAID). Enteric coating is designed to bypass initial gastric dissolution to reduce local mucosal irritation, though it does not eliminate the systemic risk of gastric ulceration.
## Primary Indications
* **Cardiovascular:** Prophylaxis of myocardial infarction (MI), ischemic stroke, and transient ischemic attacks.
* **Vascular:** Secondary prevention in patients with coronary or peripheral artery disease.
* **Inflammatory (High Dose):** Rheumatoid arthritis, juvenile idiopathic arthritis (JIA).
* **Acute Coronary Syndrome (ACS):** Initial management (generally non-enteric, chewable aspirin preferred for rapid absorption).
## Adult Dosing
* **Cardiovascular Prophylaxis:** 75–162 mg orally once daily.
* **Acute Coronary Syndrome (Initial):** 162–325 mg (non-enteric preferred).
* **Inflammatory Conditions:** 2.4–3.6 g/day divided into 3–4 doses; titrate to response or salicylate serum levels.
## Pediatric Dosing
* **JIA / Inflammatory Conditions:** 60–100 mg/kg/day divided into 4–6 doses; monitor serum salicylate levels (target 15–30 mg/dL).
* **Kawasaki Disease:** Initial 80–100 mg/kg/day in 4 divided doses until afebrile or day 14 of illness, followed by 3–5 mg/kg once daily for antiplatelet effect.
* **WARNING:** Reye’s syndrome risk. Aspirin use is strictly contraindicated in children/adolescents with suspected or confirmed viral infections (e.g., flu, varicella).
## Dose Adjustments
* **Renal Impairment:** Use with extreme caution. Avoid in severe renal failure (CrCl < 10 mL/min).
* **Hepatic Impairment:** Avoid in severe hepatic failure; increased risk of bleeding and hepatotoxicity.
## Contraindications
* Hypersensitivity to salicylates or NSAIDs.
* Active peptic ulcer disease or pathological bleeding.
* Hemophilia or other bleeding disorders.
* Children/teenagers with viral infections (Reye's syndrome risk).
* Asthma associated with "aspirin-exacerbated respiratory disease" (AERD).
## Adverse Effects
* **Gastrointestinal:** Dyspepsia, nausea, gastric ulceration, occult blood loss.
* **Hematologic:** Prolonged bleeding time, increased risk of intracranial hemorrhage.
* **Otic (High Dose):** Tinnitus, hearing loss (often reversible).
* **Renal:** Acute kidney injury, interstitial nephritis.
## Key Drug Interactions
* **Anticoagulants/Antiplatelets:** (e.g., Warfarin, Clopidogrel, DOACs) Significant increase in major bleeding risk.
* **NSAIDs:** (e.g., Ibuprofen) May interfere with the cardioprotective effect of low-dose aspirin; space ibuprofen 8 hours after or 30 minutes before aspirin.
* **Corticosteroids:** Increased risk of GI ulceration.
* **Alcohol:** Increased risk of GI mucosal injury.
## Monitoring
* **Chronic Use:** Hemoglobin/Hematocrit (occult bleeding), stool guaiac testing if clinically suspicious, baseline and periodic serum creatinine.
* **High-Dose Therapy:** Serum salicylate levels (therapeutic range 15–30 mg/dL).
* **Toxicity:** Signs of overdose include hyperpnea, tachypnea, tinnitus, metabolic acidosis, or altered mental status.
## Clinical Pearls
* **Absorption:** Enteric coating significant delays onset of action; never use enteric-coated aspirin in an acute MI setting if a chewable/plain formulation is available.
* **Compliance:** Long-term low-dose therapy is highly effective for secondary cardiovascular prevention, but adherence is critical.
* **AERD:** Patients with asthma, rhinosinusitis, and nasal polyps may experience severe bronchospasm upon aspirin exposure.
