# Ecospirin (Aspirin, Enteric-Coated)
## Overview
Ecospirin is an enteric-coated formulation of acetylsalicylic acid (ASA), a nonsteroidal anti-inflammatory drug (NSAID) and irreversible cyclooxygenase (COX-1/COX-2) inhibitor. The enteric coating is designed to minimize gastric irritation by delaying dissolution until the tablet reaches the small intestine.
## Primary Indications
* **Secondary Prevention:** Cardiovascular events (MI, stroke), TIA, and stable/unstable angina.
* **Primary Prevention:** High-risk patients (per specific clinical guidelines).
* **Acute Management:** Myocardial Infarction.
## Adult Dosing
* **Secondary Prevention/Maintenance:** 75–162 mg PO once daily. (Commonly 75 mg or 81 mg).
* **Acute MI:** 162–325 mg (non-enteric coated preferred if available) chewed as soon as possible.
* **Maximum Daily Dose:** 325 mg for antiplatelet indications. Higher doses (up to 4g daily) are for analgesic/anti-inflammatory use, but enteric-coated formulations are suboptimal for acute analgesia due to delayed absorption.
## Pediatric Dosing
* **Kawasaki Disease:** Acute phase: 80–100 mg/kg/day divided every 6 hours. Maintenance: 3–5 mg/kg/day once daily.
* **Rheumatic Fever:** 80–100 mg/kg/day divided every 4–6 hours.
* **Antiplatelet:** Not routinely indicated in children; use only under specialist guidance (e.g., cardiac post-op).
## Dose Adjustments
* **Renal Impairment:** Use with caution in severe impairment (CrCl <10 mL/min).
* **Hepatic Impairment:** Use with caution; monitor for increased bleeding risk due to coagulation factor impairment.
## Contraindications
* Hypersensitivity to salicylates or other NSAIDs (including aspirin-induced asthma triad).
* Active peptic ulcer disease or pathological bleeding (e.g., hemophilia).
* **Pediatric population:** Do not use for viral infections (flu-like symptoms or chickenpox) due to risk of **Reye’s Syndrome**.

* **NSAIDs (Ibuprofen, Naproxen):** May attenuate the antiplatelet effect of aspirin (take aspirin at least 30 minutes before or 8 hours after ibuprofen).
* **Corticosteroids:** Increased risk of GI ulceration.
* **SSRI/SNRIs:** Increased risk of upper GI bleeding due to platelet inhibition.
## Monitoring
* **Baseline:** Hemoglobin, Hct, renal function (SCr).
* **Ongoing:** Signs of occult or overt bleeding (melena, hematemesis, bruising), gastric discomfort.
* **Salicylism:** Monitor for tinnitus, confusion, or hyperventilation if high doses are used.
## Clinical Pearls
* **Absorption:** Enteric coating slows absorption, making daily maintenance appropriate, but it renders the drug unsuitable for acute relief of ischemic chest pain where rapid onset is required.
* **Switching:** If a patient is on daily aspirin for cardioprotection and requires regular NSAIDs for pain, consider the potential for pharmacodynamic interaction.
* **Discontinuation:** Evidence suggests a rebound pro-thrombotic state upon acute cessation; do not stop therapy without medical supervision.
