# Ecospirin (Aspirin, Enteric-Coated)
## Overview
Ecospirin is an enteric-coated formulation of acetylsalicylic acid (ASA), a nonsteroidal anti-inflammatory drug (NSAID) and irreversible inhibitor of cyclooxygenase (COX-1 and COX-2). Enteric coating delays dissolution until the small intestine, intended to reduce direct gastric mucosal irritation, though it does not eliminate the risk of systemic gastrointestinal ulceration.
## Primary Indications
* Prophylaxis of myocardial infarction (MI) and ischemic stroke.
* Acute coronary syndromes (ACS).
* Management of stable or unstable angina.
* Secondary prevention of cardiovascular events.
## Adult Dosing
* **Acute Coronary Syndrome:** 160–325 mg chewed immediately, then 75–160 mg daily.
* **Secondary Cardiovascular Prevention:** 75–162 mg daily.
* **Maximum Dose:** Generally 325 mg/day for chronic maintenance. Higher doses (up to 4 g/day) are rarely used for anti-inflammatory purposes due to toxicity risks, and enteric-coated formulations are suboptimal for rapid onset in acute settings.
## Pediatric Dosing
* **Kawasaki Disease:** 80–100 mg/kg/day divided every 6 hours (acute phase), then reduced to 3–5 mg/kg/day (maintenance). Always follow institutional protocols.
* **General Note:** **Avoid** in children/adolescents with viral infections due to the documented risk of **Reye’s Syndrome**.
## Dose Adjustments
* **Renal Impairment:** Use with caution in severe impairment (CrCl < 10 mL/min).
* **Hepatic Impairment:** Avoid in severe liver disease; increased bleeding risk and potential for worsening hypoalbuminemia.
* **Geriatric:** Use lower end of the dosing range; increased risk of GI bleed.
## Contraindications
* Hypersensitivity to salicylates or other NSAIDs.
* Active pathologic bleeding (peptic ulcer, intracranial hemorrhage).
* Asthma with nasal polyps/rhinitis (risk of exacerbation).
* Children/teenagers with viral illness (Reye’s syndrome risk).
* History of GI bleeding.
## Adverse Effects
* **Common:** Dyspepsia, nausea, abdominal pain.
* **Serious:** Gastrointestinal ulceration/hemorrhage, occult blood loss, hypersensitivity reactions (bronchospasm), salicylism (tinnitus, tachypnea, metabolic acidosis).
## Key Drug Interactions
* **Antiplatelets/Anticoagulants:** Increased bleeding risk (e.g., warfarin, clopidogrel, DOACs).
* **NSAIDs:** Ibuprofen may interfere with aspirin’s antiplatelet effect if taken concurrently (administer aspirin at least 30 minutes before or 8 hours after ibuprofen).
* **Corticosteroids:** Increased risk of GI ulceration.
* **SSRIs:** Increased risk of bleeding due to inhibition of serotonin reuptake in platelets.
## Monitoring
* **Baseline:** CBC (hemoglobin/hematocrit), occult blood test, renal function.
* **Ongoing:** Signs of GI bleed (melena, hematemesis), persistent abdominal pain, tinnitus (marker of toxicity).
## Clinical Pearls
* **Enteric Coating:** Do not crush or chew enteric-coated tablets; this destroys the coating and increases gastric irritation.
* **Acute Situations:** For acute MI, a non-enteric-coated (plain) aspirin is preferred because it dissolves and absorbs faster.
* **Antiplatelet Effect:** Irreversible inhibition of COX-1 lasts for the lifetime of the platelet (7–10 days).
