# Ecospirin (Aspirin, Enteric-Coated)
## Overview
Ecospirin is an enteric-coated formulation of acetylsalicylic acid (ASA), an irreversible inhibitor of cyclooxygenase-1 (COX-1). Enteric coating reduces direct gastric mucosal irritation, though it does not eliminate the systemic risk of gastrointestinal (GI) ulcers.
## Primary Indications
* Secondary prevention of myocardial infarction (MI) and ischemic stroke.
* Acute coronary syndrome (ACS).
* Prophylaxis of thromboembolic disorders in high-risk patients.
* Management of stable/unstable angina.
## Adult Dosing
* **Secondary Prevention (CVD/Stroke):** 75 mg to 162 mg orally once daily. Standard clinical practice often favors 81 mg daily.
* **Acute Coronary Syndrome:** 162 mg to 325 mg (non-enteric preferred if chewable) as a single loading dose, followed by 81 mg to 162 mg daily.
* **Maximum Dose:** Generally 325 mg daily for antiplatelet therapy. Doses up to 4g/day are used for analgesia/anti-inflammatory purposes (though rarely via enteric-coated formulations).
## Pediatric Dosing
* **Kawasaki Disease:** Acute phase: 80–100 mg/kg/day divided every 6 hours. Maintenance: 3–5 mg/kg/day once daily.
* **Rheumatic Fever:** 80–100 mg/kg/day divided every 4–6 hours.
* **Antiplatelet:** Generally not indicated unless directed by a specialist for specific cardiac anomalies.
* *Warning:* Due to the risk of **Reye’s Syndrome**, aspirin is strictly contraindicated in children and adolescents for viral-like illnesses (fever/influenza).
## Dose Adjustments
* **Renal Impairment:** Use with caution; monitor for increased bleeding risk. Avoid in severe renal failure (CrCl < 10 mL/min).
* **Hepatic Impairment:** Use with caution; avoid in severe liver disease due to potential for coagulopathy and worsening of gastric mucosal integrity.
## Contraindications
* Hypersensitivity to salicylates or NSAIDs.
* Active pathologic bleeding (e.g., peptic ulcer, intracranial hemorrhage).
* Asthma with nasal polyps (risk of severe bronchospasm).
* Children/adolescents with viral infections (Reye’s Syndrome).
## Adverse Effects
* **Common:** Dyspepsia, nausea, abdominal pain.
* **Serious:** Gastrointestinal bleeding, peptic ulcer disease, hypersensitivity reactions (anaphylaxis), and increased bleeding time.
## Key Drug Interactions
* **Anticoagulants/Antiplatelets (e.g., Warfarin, DOACs, Clopidogrel):** Significant increase in hemorrhage risk.
* **NSAIDs (e.g., Ibuprofen):** May interfere with the cardioprotective effects of low-dose aspirin; space dosing (administer aspirin 30 minutes before or 8 hours after ibuprofen).
* **Corticosteroids/SSRIs/SNRIs:** Increased risk of gastrointestinal ulceration/bleeding.
## Monitoring
* **Clinical:** Assess for signs of occult bleeding (melena, hematemesis, epistaxis, unexplained bruising).
* **Laboratory:** Periodic hemoglobin/hematocrit if long-term high-dose therapy is used; renal function (BUN/SCr) in vulnerable populations.
## Clinical Pearls
* **Absorption:** Enteric coating delays absorption. In an acute emergency (e.g., suspected MI), use **non-enteric coated** aspirin if available for faster onset.
* **Compliance:** Do not crush or chew enteric-coated tablets, as this destroys the delayed-release mechanism and increases gastric irritation.
* **Surgery:** Generally held 5–7 days prior to elective surgery depending on the procedure and surgeon preference; consult the prescribing physician.
