# Ecospirin (Aspirin, Enteric-Coated)
## Overview
Ecospirin is an enteric-coated formulation of acetylsalicylic acid. The enteric coating is designed to minimize direct gastric mucosal irritation by delaying dissolution until the tablet reaches the small intestine. It acts as an irreversible inhibitor of cyclooxygenase-1 (COX-1), effectively preventing platelet aggregation for the lifespan of the platelet (7–10 days).
## Primary Indications
* Prophylaxis of myocardial infarction (MI) and ischemic stroke.
* Management of acute coronary syndrome (ACS).
* Secondary prevention of cardiovascular events.
* (Labeling may vary by region; verify local regulatory approval for specific analgesic/anti-inflammatory indications).
## Adult Dosing
* **Secondary Cardiovascular Prevention:** 75 mg to 325 mg orally once daily. Long-term maintenance is commonly 75–100 mg daily.
* **Acute Coronary Syndrome (ACS):** Initial loading dose of 160 mg to 325 mg (non-enteric coated preferred for initial loading if available; chew/crush to speed absorption).
* **Analgesic/Antipyretic (where indicated):** 325 mg to 650 mg every 4 to 6 hours as needed; maximum 4 g per day.
## Pediatric Dosing
* **Kawasaki Disease:** Acute phase (anti-inflammatory): 80–100 mg/kg/day divided every 6 hours (max 4 g/day). Maintain for 48 hours after fever subsides, then reduce to antiplatelet dose.
* **Antiplatelet/Thrombosis:** 3–5 mg/kg once daily.
* **Warning:** **Avoid in children/adolescents with viral infections** due to the high risk of **Reye’s Syndrome**.
## Dose Adjustments
* **Renal Impairment:** Use with caution in severe renal impairment (CrCl <10 mL/min).
* **Hepatic Impairment:** Avoid in severe hepatic failure.
* **Geriatric:** Increased risk of GI bleeding; assess bleeding risk vs. benefit.
## Contraindications
* Hypersensitivity to salicylates/NSAIDs.
* History of GI bleeding or active peptic ulcer disease.
* Bleeding disorders (e.g., hemophilia, von Willebrand disease).
* Children/teenagers with viral illness (due to Reye's Syndrome risk).
* Asthma triad (aspirin-sensitive asthma).
## Adverse Effects
* **Common:** Dyspepsia, nausea, abdominal pain.
* **Serious:** Major gastrointestinal hemorrhage, intracranial hemorrhage, hypersensitivity reactions (anaphylaxis), and increased bleeding time.
* **Toxicity:** Salicylism (tinnitus, hyperventilation, metabolic acidosis, confusion).
## Key Drug Interactions
* **Anticoagulants/Antiplatelets (Warfarin, NOACs, Clopidogrel):** Synergistic effect significantly increases bleeding risk.
* **NSAIDs (Ibuprofen):** May interfere with the cardioprotective antiplatelet effect of low-dose aspirin. If taken together, dose aspirin at least 30 minutes before or 8 hours after ibuprofen.
* **Alcohol:** Increases risk of gastric mucosal injury.
* **Corticosteroids:** Increased risk of GI ulceration.
## Monitoring
* **Baseline:** CBC (hemoglobin/hematocrit), occult blood test, renal function (SCr).
* **Ongoing:** Monitor for signs of overt bleeding (melena, hematemesis, bruising) and symptoms of salicylism. Routine monitoring depends on the indication and duration of therapy.
## Clinical Pearls
* **Enteric coating delay:** Enteric-coated formulations are **not** recommended for the initial management of acute MI because of delayed absorption. Use non-enteric tablets in acute settings.
* **Bioavailability:** Food may reduce the rate—but not the extent—of absorption.
* **Surgery:** Generally held 5–7 days prior to elective surgery due to irreversible platelet inhibition; consult surgical protocols/guidelines.
